Brief Answer:
Dynamic x-rays are necessary.

Detailed Answer:
Hello and thanks for using HCM.

I have read your question and understand your concerns.

Regarding MRI of your brother's spine, it is evident that there is spondylolysthesis at two levels ( slipped vertebrae ), and degenerative spine disease with significant compression on neurological structures.

In such cases, dynamic x-rays of lumbosacral spine ( neutral, flexed and extended position ) are necessary to understand correctly the degree of slipping.

If there is significant instability and there is no improvement after conservative treatment ( painkillers, physical therapy, epidural injections ) decompression and fusion surgery of the affected levels of the spine is necessary.

Get an opinion from a Neurosurgeon or spine Specialist about overall condition of your brother.

Hope you found the answer helpful.

Kind regards.
