Question: My father is 72 yrs old and is diagnosed with Parkinson's plus. He was recently admitted to Sakra hospital due to an infection and was discharged after a week of antibiotic treatment etc...Due to his condition his speech is not very good and is unable to communicate. From last 3-4 days he seems to be in severe pain while passing urine frequently. He has been circumcised and there seems to be no infection from the outside. Please advice.

Thanks for your query ,based on the facts that you have posted it appears that your 72 years old father was admitted in hospital for Parkinson's disease and was circumcised for urinary complains. Now he is has severe burning while passing urine and frequency for urination .

Looking at his age his urinary complains are most likely to be due to UTI secondary
to bladder outlet obstruction due to Enlarged Prostate .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Was he investigated to rule out Enlarged Prostate .?

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Uroflometry.

In the mean while give him take broad spectrum antibiotics like Cefotaxime along with urinary antiseptic like Nitrofurantoin twice daily and Tomsulosin once daily with urine alkalizer thrice daily .Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure the drinks more water .To keep his urine dilute This will help to control dysuria.

Further treatment will depend upon result of these tests and final diagnosis.

Hello Dr. XXXXXXX Thank you for quick response. Please find my comments below.

1> Recently he was admitted to hospital due to high blood sugar levels and low oxygen levels. Later through diagnosis it the reason was found due to an infection causing blood sugar levels to shoot up. Blood culture reports was found to be -ve with the antibiotics administered.

2> I think some x-rays were done and nothing +ve ws found although I am not sure the objective of objective of line of investigation for those x-rays.

3> Circumcision was done some 15-20 yrs back when he was diagnosed with Diabetes.

For more clarity, I will scan recent discharge summary and will upload for your review and further advice.

In the meantime, as per you confirmation, we will give him antibiotics like Cefotaxime along with urinary antiseptic like Nitrofurantoin twice daily and Tomsulosin once daily with urine alkalizer thrice daily .

But not sure for how many days ? Can you please confirm.

I will also consult our family physician for next steps.
