Question : I have been noticing small dots near my penis I ignored at a first glance. But after a 3-4 weeks I noticed that they are spreading silently. It started near the veins as seen in the picture. But now it is spreading circularly small line of dots are developing(As shown by black marker). This affected area is normally under the skin when penis are not exited. This area only comes out when I masturbate. There was no itching for last 4 weeks but I am feeling slight itching since last 3 days especially when I am working out in a gym and when I am sweating a lot. Not to much itching but slight enough to distract me and think about it. I had a protected sex 2 months ago. However, my partner gave me a blow-job without condom. I am not sure about her. I check HIV after 2 months of protected intercourse. I am HIV negative. I am not sure what is the problem. I would be glad if I can get advise from you.
Thank you.

Brief Answer:
Fordyce spots/glands: prominent normal oil glands

Detailed Answer:
Hello,

Welcome and thanks for posting your query to the forum.

I can understand your apprehension for the bumps over the penile area. It is really stressful for a man to have anything unusual on genital area.

Now according to your query you have the problem of small bumps over undersurface of penis.

After reading your query with diligence and analyzing the pictures with best clinical acumen let me reassure you that your condition is simple “Fordyce spots/glands”.

To tell you medically about these bumps: these bumps are most commonly the spots on the undersurface of penis. These are pinhead size or slightly larger bumps present over mainly the under surface of the penis i.e. on the frenulum area. There is no correlation of these bumps with any type of Sexually Transmitted Diseases, so just relax.

You might be wondering that how and why these are formed!!!

These are formed due to proliferation of the normal oil glands which are present over this area. Any young men can get these in minor or major form after the age of 16 years due to over-activity of oil glands at your age. These are totally asymptomatic (without any problem).

These are more prominent when the penis is erect and less in lose state of penis. These are most commonly confused with genital warts in their initial course of appearance. A close examination is the must to differentiate one from another.

Thanks a lot for your good resolution pictures that I can confirm these as Fordyce spots. There is no requirement of any treatment in this condition as it is totally normal condition. If at all some person are panicky then we do remove these with radiofrequency ablation.

I hope these information's will help you. If you have further queries I will be happy to help.

Brief Answer:
No medication to prevent its further formation

Detailed Answer:
Dear young fellow,

Welcome again and thanks a lot for further swift follow up.

As I said these are just over proliferation of normal glands and can occur in anyone. There is no medication to prevent its further formation and I personally suggest you to not to get anxious and apply anything over or near it. There is nothing which can prevent the further formation. The penile skin is such a thin skin that if you try and apply one or other thing then rather than helping can harm your soft penile skin.

If at all you are worried then can get it removed by machine called as Radio-frequency by a experienced dermatologist. But in my set of patients with your kind of issue I suggest all of them to let it be as it is. These can slightly increase in size but can never cause any problem in normal or sexual functions.

So just relax and do not worry.

I hope these informations clears all your doubts. If you have further queries I will be happy to help or if not then can close the discussion an please rate the answer.

Thank you for your response.
I am still anxious, and the reason is it is spreading, and in a specific pattern. It is itchy sometime. I am conscious because what if it will cover the whole portion. since as you told this is not a contagious disease. But, it is not a random place it is spreading in a line. And after that, the line is getting thicker and thicker. It is growing one by one, next to the previous one and next and next. I hope you can give me an elaboration of this thing to happen.

Thank you.

Brief Answer:
Seen in one line due to its pattern of formation.

Detailed Answer:
Hello young buddy,

Welcome again and thanks for your further follow up.

I can very well understand your part of concern and as a young man can feel the anxiousness.

The most important and reassuring part of your problem is that it is simple physiological change and not a disease entity.

So first and foremost you should relieve all your anxiety related to this condition. You are having this in one line because of its pattern of formation. it can be in bizarre form or can be in a pattern as you have.

I already told you that due to simple over-activity there is bit prominence. It will not involve the complete penile skin and therefore not cover the complete penis.

The mild itching is just because of your over-attention and continuous mental pressure of something abnormal down under.

As I see you are stressed with its appearance so you can visit a good dermatologist and can get it removed within few minutes. But as I already said it will never going to cause any problem in your whole life so can be left as it is also.

So just relax and do not worry.

I hope these informations clears all your doubts. If you have further queries I will be happy to help.
