What Causes Small Tumor In Uterus And Heavy Period Bleeding?

Question: I am 36 years old n was diagnosed with a very small tumour in my uterus since one year. I used to have heavy bleeding during my menstrual periods but due to the very small size of the tumour my Gynaecologist had suggested to go for a D&C followed by insertion of Mirena (IUD). This procedure was done on 10th Mar'14 under anaesthesia. After the procedure I had moderate bleeding(accompanied with abdominal pain) for 3-4 days followed by spotting for another 2-3 days. On the eighth day there was substantial abdominal cramps followed by bleeding. Since then the bleeding has not stopped. On doctor's advice I had started taking Tranostat-MF (3 tablets daily) from 23rd Mar'14 onwards, but that was also ineffective. Today he has prescribed Regestrone 5 mg (twice daily) for next 25 days and I have already taken one dose, still the bleeding has not stopped. Because of this continuous bleeding I am gradually getting weaker, also I am a working lady in a core industry having hectic schedule.My query is whether Mirena has been ineffective for me and also if Regestrone shall work in my case. If not what is the ultimate solution at present? I am desperately in need of stopping the bleeding right now.Please advise.

Brief Answer: continue with regesterone. Detailed Answer: Dear XXXXX, Thanks for writing to healthcare magic. I can understand your concern. Prolonged bleeding is very tiring. I presume the small tumor you mentioned is a fibroid in the uterus. Mirena acts by releasing a hormone inside the uterus which will suppress the endometrial lining over the time and make it thin so that after sometime you will have very scanty bleeding or absent periods. The mirena being inserted recently should be given time. It's too early to say it's ineffective. The tranostat Mf should have worked but as you say you are continuing to bleed and now you are taking regesterone you should give the Drug some time. I would recommend increasing the dosage to thrice a Day. Please discuss with your doctor. Also you can take tranostat Mf along with regesterone. After 3-4 days you will see results. If you stop regesterone you will have recurrance of bleeding. If regesterone also doesn't work than ovral g can be tried. I hope you are taking iron supplements and multivitamin. Thanx a lot for ur reply....Can i increase the dose of regestrone after 2-3 days?Please suggest some supplements-iron n vitamins....

Brief Answer: increase the dose now and taper the dose later. Detailed Answer: Dear XXXXXX, Thanks for writing back. The proper method is to start with higher dose. Then after 3-4 days when the bleeding stops we taper the dosage. To patients in my clinic with similar complaints I have prescribed tab regesterone two tablets twice a day for 3-4 days with excellent results. After 3-4 days I review to check whether there is good response. I think you should discuss with your doctor regarding increased dose to 3 or 4 tablets per day. Tab orofer-XT and becosules -z each once a day after Food for iron supplementation. Please write back after 4 days to review the treatment. Also you mentioned d&c was done. Please attach the pathology report of the d&c sample next time. Thanks Dr Bhagyashree

Dear Madam, In reference to my previous queries, after having taken Regestrone 5mg for 6 days, the bleeding has got controlled but still its there with occasional abdominal cramps. After the cramps/pain recedes, normally there is bleeding. Since last 2-3 days while urinating, the stuff is coming out mostly rather than soiling of pads. But my querry is how long shall I need to carry this? Its really getting frustrating, also I need to go on official tour for 2-3 days from 2nd April onwards. I am afraid whether during journey I'll have problems. I had talked to me doctor regarding increase of Regestrone dose but he had suggested to consume the same dose for 25 days.I am attaching the histopathology report of my endometrial curettings. Brief Answer: tab meftal spas for cramps, continue regesterone. Detailed Answer: Dear XXXXXX, Thanks for writing back. The intermittent bleeding you are experiencing is irritating but harmless.Tthe little bleeding which is high in vagina continues to trickle out over few days. I would add meftal spas tablets for cramps. You can discuss about it with your treating doctor. Please continue to take regesterone as advised. if you stop now bleeding will recur. I couldn't find the report you said you have attached. If its difficult to do so you can type the final impression mentioned in the report. Travel wont cause more bleeding as long as you take the regesterone. Thanks Dr Bhagyashree
