1. Since how long are you having this? Is this an acute problem or Chronic one
2. Do you have any known allergy?
3. Any Cough , Fever, Eye redness associated?
4. Any Mucous production?
5. Give me details about the medical history of hay fever that you have mentioned.
6. What medications you are taking, please enlist.
7. Attach if any investigations, new or old you have.
