Brief Answer:
Hematoma formation due to soft tissue injury.

Detailed Answer:

Hello,

The black and blue coloration at the back of the hand suggests an internal bleeding and hematoma formation. This can happen due to an injury. The soreness can be due to a soft tissue injury or a bone injury in the area. This needs a direct examination and X-ray to detect the extent of injury and to rule out a bone fracture. Do consult your physician and give rest to the hand. You can take a mild pain killer after meals.

I hope this answers your query. In case you have additional questions or doubts, you can forward them to me, and I shall be glad to help you out. Wishing you good health.
