What Causes Soreness And Tightness In The Hip?

Question: Hi, Doctors

It has been 5 days , Last Friday I began feeling some soreness and tightness in my Right Hip such as generalized pain 6/10. Its not Tuesday and I still feel stiffness , ache feeling, and heaviness 7/10 uncomfortable. I am 34 years old active but some what overweight for my height and weight 5'11 220 pounds. When I palpate my hip I can feel pain on the medial/ anterior parts like glut medius/ minimus and rectus femoris area close the the groin and adductors and psoas flexors. I do not feel pain with weight bearing or bending, mostly during sitting ill get a ache and stiffness feeling not necessary painful or sharp. I have not heard any popping or grunting noise when walking either just lots of stiffness and aches. My range of motion seems normal and not necessary painful just ache. My question is can this be osteoarthritis or any other arthritic condition? Or can this be a muscle strain or some other orthopedic condition I am not aware off?

I have gone through your query in detail and understand your concern.

I generally see many cases like this at my clinic.I give them a splint,rest,analgesics,anti-inflammatory tablets and ice pack till the swelling reduces to some extent. Then i get an xray and if necessary an mri done to find out the exact reason.

In your case,there are 3 possibilities-
POSSIBILITY 1. SOFT TISSUE INFLAMMATION- there may be no bony involvement,only the soft tissues around the back might be inflammed that may be causing the pain

POSSIBILITY 2. NEUROPATHY- there may be a partial or complete swelling of the nerves in that region that may be the root cause of such kind of pain.

POSSIBILITY 3. DISC PROLAPSE-there may be a compressed a nerve in the region somewhere that might be causing radiating pain in those regions.Possibility of a disc prolapse should be kept in mind.


I suggest you get an Xray as well as an MRI bone to find out the exact cause.
Physiotherapy,analgesics,anti-inflammatory drugs should help.


But i assure one thing, even if it is either of the above 3 possibilities,all have a good prognosis and you should eventually get back to normal in coming few days.Nothing to worry about.Do get back to me after ur reports are ready.

Hi, Dr Mahaveer

Fist off, I would like too thank- you for the very clear and informative response to my question.

Today my right Hip was feeling fairly stiff and tight all day , and also experienced some generalized ache and tightness in my lower back that radiated to my left side. Most of my stiffness is around my right hip glut medius area almost feels like a bruise but maybe because i been palpating it aggressively. I went to the gym today ran for 30 min at moderate speed and did the optical for 30 min moderate rate, this seemed to loosen my hip and feels better.Is this okay to do?

My question to you doctor is; what makes you think this is something other then arthritis symptoms?

There are a few things that you mentioned, that made me think that this is definitely something than arthritis-

1. Arthritis is generally localised pain, yours is generalised.

2. Arthritis pain increases on standing, walking and weight bearing, you get pain while sitting

3. Hip range of motion is restricted in arthritis. In your case range of movements is not affected.
I am not saying that you dont have an arthritis, but I felt that we should also be aware of the other possibilities as well, so that they dont get neglected.

We can keep both differential diagnosis in mind- arthritis and non arthritic conditions but definitely your symptoms need evaluation and should not be neglected.

i suggest you to stop gym and do only certain stretching exercises till we reach a diagnosis.
