Let me first reassure you, things can be taken care.
Coming to complaints of pain in abdomen, i assume you meant Epigastric region when you said top of abdomen. Pain which aggravates after eating is possibly to be due to stomach pathology. At your age group its likely to be due to Gastritis/Gastric Ulcer. Gastritis is inflammation of the mucosal lining of the stomach.
Bleeding which is red and more than stools and if painless is likely to be due to
1. Hemorrhoids/piles: More likely
2. Colitis
3. Growth in colon: Less likely , as your complaints are present for more than 18 months.
Exact diagnosis will depend on clinical examination.
I would like t know additional details about your complaints
1. Did you experience such pain abdomen on previous occasion?
2. Do you have vomiting?
3. Do you have
constipation /pain while passing stools
4. Do you have vomiting / distension of abdomen.
You will require thorough reassessment by
General Surgeon / Gastroenterologist for further evaluation and will require
1. Digital rectal examination
2. Colonoscopy
3. Upper gastro-intestinal
Endoscopy : if te pain in abdomen does not respond to anatacids
In the meantime , I would suggest you the following which will definitely help
1. Avoid spicy/oily food
2. Avoid alcohol in any form
3. Antacid: Pantoprozole capsule 40 mg once daily early morning wih glass of water will be helpful . It suppresses
gastric acid secretion and will be very much beneficial in managing the pain.
4. Plenty of oral fluids: Ensure water intake of 2-3 liter a day
5. Laxative like
Lactulose will be very helpful in managing bleeding.
