Posted on Mon, 14 Apr 2014
Question: Hello. A few years ago I was infected with Helicobacter Pylori. A month and a half ago I went to an asian restaurant (not too much higienic) and I eat a lot of food: chicken, pork and beef. 3 hours after I ate this meal I suddenly felt a hot and pain in my stomach, and I had suddenly diarrhea with green stools. I have pain between my shoulder blades and stomach pain. The Doctor made me a breath test, and it was negative to Helicobacter Pylori. But I have a lot of pain in my stomach, specially in the mornings, I also have green stools diarrhea. I have done an ecography and a blood analysis and Pancreas, Liver, kidneys and Gallblader are ok. What can I have?. Thank you.

Brief Answer: Examination of stool is needed.... Detailed Answer: Hi, I have gone through your medical history and feel sorry for the situation you are in. Having pain and diarrhea after eating at the Asian restaurant, which is persisting until now, is related to: - possible bacterial infection of the bowels - triggering/aggravation of gastric problems (as Asian foods are really hot and chilly) I suggest to run further tests to diagnose your real problem: - culture of feces for bacteria - examination of feces for ova/parasites - upper endoscopy to evaluate stomach and upper duodenum I am happy that your H.pylori test resulted negative. Even though it would be positive, it is not characterized by frequent and green stools you have. Until the test results, I advise to: - start taking PPIs (omeprazole, pantoprazole, etc) - take antiacids - have plenty of fluids (water, soups) - have light meals (avoid chilly, fatty foods) - take tasectan for diarrhea - take probiotics to stabilize intestinal flora Hope it was of help! Thank you very much for your explanation. One more question: is it possible that I have a gastrinoma? or stomach cancer?, I am very afraid about that. This friday I have an endoscopy.

Brief Answer: Wait until endoscopy results... Detailed Answer: Hi, Your clinical symptoms are not typical for gastrinoma (stomach cancer). Do not worry to much. Please have patience until you get the upper endoscopy results. They will clarify your real problems. Looking forward to have your endoscopy results for second opinion! Dr.Klerida

First of all thank you very much for your answers. I have already the result of gastroscopy: esophagus, stomach, duodenum all normal I asked to the doctor about my pain and he answer me that at this precise moment I donĀ“t have gastritis, I asked to him that if gastritis can come and go and he told me: "yes". So the result is normal, but I still have a great pain after meals, and this pain last for hours, and I still have green stools. I am waiting the result of stool analisys. So everything points to a possible bacterial infection of the bowels. I have a feces culture test the next week. Can a infection of the bowels lasts almost two month?. Can I take omeprazol?, I have read that omeprazol can worse bacterial infections in some cases. Thank you very much again.

Brief Answer: Wait until stool tests results... Detailed Answer: Hi, I am happy to know that you have already done the examinations of stool. It is true that a bowel infection can last for that long if not treated properly in time. It is also true that omeprazole can cause imbalance of intestinal flora due to increase of stomach pH. To my opinion, you should not use omeprazole as long as the upper endoscopy resulted normal, and you do not have gastritis for the moment. I highly suggest to wait until stool tests results for specific treatment. You can forward the reports to me for further evaluation. Dr.Klerida

Thank you very much. You have seen very kind and gentle to me. More questions: The pain in abdominal area could be pain in the bowels instead of stomach?. I feel bloated sometimes, could it be produced by bowels?. if gastritis can come and go could I have gastritis in some moments of the day, for example: after meals, despite de fact that in gastroscopia it was not gastritis at this moment?

Brief Answer: Could be bowel-related.... Detailed Answer: Hi back, Thank you for following up. The pain in abdominal area could be related to bowel problems too. The diagnostic tests will determine the real origin. Feeling bloated is related to both stomach and bowel problems. Gastritis does not come in the morning and disappear in the evening. Its signs will persist for some times (days to months), then, will improve. Let me know if you have further queries! Dr.Klerida

Only one last question and thank you very much for your patience with me. You have told me this: "Gastritis does not come in the morning and disappear in the evening. Its signs will persist for some times (days to months), then, will improve." So, since I have not gastritis visible in my endoscopy , do you think that it's signs will persist for some times for me until it improve?. Thank you very much. I have to tell you that I have one more symptom : in the last days I have developed lactose intolerant. Before my "posible poisoning" in the asian restaurant I could drink a lot of milk without any problem, after that, I cannot drink milk because I will defecate it after a few minutes in a liquid stool.

Brief Answer: Temporary bowel dysfunction... Detailed Answer: Hi, Thank you for following up. Since gastritis was not visible in your endoscopy, then, all the symptoms you are having are due to food poisoning. Developing lactose intolerance is also associated with food poisoning. The intestinal structure and function have been altered and it will take some time until everything will be back to normal again. Avoid taking foods that might cause frequent bowel motions.
