Question: My 66 year old wife has a stomachache and diarrhea, she is feeling chills and has cold hands and feet. These symptoms started on the drive home from dinner with friends. She normally warm natured. She had steak, baked potato, salad and brusselspruts baked with cheese.

I appreciate your concern

Based on the history you described, it appears to be gastroenteritis. Blood in stool, high grade fever with raised TLC or white blood cells count on a complete blood count would point towards infectious diarrhea and require prompt therapy with antibiotics. Dehydration and electrolyte imbalance must be prevented. ORS/Gastrolyte etc can be given orally if there is no associated vomiting and oral intake is normal. If oral intake is not possible or the blood pressure is very low and there are signs of severe dehydration then IV fluids would be required urgently. It would be a good idea to go to an ER near you in that case.

The chills are possibly due to the hypothalamus response struggling to keep the body warm.

The cold feet can be related to narrowing of the vessels perfusing the limb's skin due to fluid loss and hypotension shunting the blood to vital organs.
