Question: What would cause a person to just all the sudden wake up one day and start obsessively questioning why we're all here. Not feeling real so to speak.

The problem you mentioned can occur in following situations.

Person is known case of high blood pressure and had episode of transient ischemic attack.

she/he may be undiagnosed case of hypertension and sudden rise in blood pressure there.
,
Any infarct in brain may be chronic or recently developed.

Other causes of delirium(disorientation) like electrolyte disturbance , substance use, fever, any injury.. so on.

All above are direct physical causes.

There may be psychological causes:

Sever stressful situation, history of any psychotic disorder, following of any religious path in which searching for cause of life is there or person can imitate the same with or without any secondary gain.

I would like to know fro you following:

Who has suffered from this? Are you or someone else? Describe detail history in terms of personal, past, family, medical and substance related of the suffering person.

Detail about episode:

Since when , last for, any other symptoms during episode,frequency etc?

It's myself I'm asking this question for. Oddly enough it all started happening when I got pregnant with my second child. I'm a very healthy non drug user, no drugs of any sort in my environment. It started with a anxiety feeling in my chest that would not stop and when I would start to fall asleep I would jump and gasp for air. I felt dizzy often farther along in the pregnancy I started feeling not real I would drive down the road and look at the environment around me and think (why are we all here) why am I here? I would be eating and look down at my plate and it would like look huge to me! I started seeing spots in my vision black spots like bugs flying around, and one day I got sick and when I vomited as I raised up I saw firework type flashes of light in my eyes. I went to the Dr they had zero answers for me other than to tell me my blood platelets were low and they didn't know why. Never heard anything else about that. I function fine I'm not crazy I'm aware this is happening to me and aware something isn't right.

My first confusion is that you have mentioned in profile 42 year old male and you told your problem is started with second pregnancy! I am assuming right now that you are female.

Symptoms you mentioned are suggestive of anxiety disorder and pregnancy is the precipitating factor. In pregnancy, lot of hormonal and physiological changes occur. So pregnancy is a different stressful physiological state of body. Sometime due to drastic change in hormones woman feels anxiety and weird feeling like you felt. It will go away with counseling and relaxation or may be some anti anxiety medicine needed.

To help you more please provide following detail:

Since when are you pregnant?

In which month of pregnancy problem occurred?

The symptoms you have mentioned are occurring daily or intermittently or only once?

Send me your report of blood which you are talking about in which platelets are decreased.

The account is in my husband's name. My apologies. It started the end of October In 2012 I found out I was pregnant in November 2012. The feelings I explained were pretty much non stop all day every day! I didn't want to be left alone at home because of it. I don't have my blood report when that issue came up at the doctor I was about 7 months pregnant and they acted like it was no big deal when I went in labor the explained mine was low and couldn't have a epidural however I never progressed and they made me have a c-section. After I had my son August 2013 it got worse I even had a episode blurred vision, dry heaving etc where my family called an ambulance but they found nothing wrong at the hospital.

It's ok, I got the point. No need to apologize. You seem to be suffering from some kind of anxiety disorder. It may be generalized anxiety disorder, panic disorder, obsessive compulsive disorder or post traumatic disorder.

However as it is started in pregnancy, I would like to go for hormonal evaluation FSH and LH and Thyroid as well as MRI brain to rule out any organic factor, if I were your treating doctor.

For diagnosis, detail history in terms of personal, past, family and medical history; physical and neurological examination and mental status examination needed.
I am somewhat surprised that why are you not referred to psychiatrist yet by your doctor.

You can be treated with psychotherapy and/or medicines.

Psychotherapy: CBT- Cognitive Behavior Therapy would be specifically helpful.

Medicines: Anti anxiety medicine like clonazepam and SSRI(selective serotonin reuptake inhibitor) group of medicines like fluoxetine are quite helpful.

That makes sense. I lost my mother had a divorce and custody battle all in the same year I got pregnant. Is it possible to recover and not be on medication I do not want to be medicated

Sorry for mother and divorce. Because of these social circumstances you would be in more stress.
To judge whether you would be fine without medicine or not, require detail clinical evaluation by a psychiatrist.
Pyschodynamic psychotherapy and supportive psychotherapy would be helpful. And you may be cured without medicines.

But if your situation is so that you need to be relax for psychotherapy , some anti anxiety medicines may be needed.

And overall if it is not possible to move forward without medicines by chance, please do not hesitate to take as improvement and stability matters for healthy life.
