Question : Hello, I have history of enlarged prostate and I currently have difficulty passing feces. I am also experiencing sudden block outs. This 'block out' occurs suddenly, for instance, if I am walking i will suddenly freeze and I lose control over my body so if I have something in my hand it will fall to the ground and my feet move with a trembling/jerking motion and my family members have to hold me up. When this episode occurs, sometimes my eyes roll over and my brain seems to be mixing up as my speech is slurred and i talk random things (but this is just temporary). When this is over i remember it happening and I do not have any memory loss. I am really concerned about the prostate though as I feel like it is blocking off my passage so i have to strain to pass feces. I went to the doctor last week Saturday and explained my situation but the doctor said he was more concerned about the blocking out and ordered a blood test, ECG test and a brain CT scan. I completed the ECG test and blood test and got back the results yesterday. I took them to the doctor today and was told that the ECG is normal. However, I was not too comfortable with how he explained the blood test results. I have uploaded the blood test results and would like you to explain to me what those results mean. Also, the doctor said these tests did not show anything regarding what could be causing the block outs and that I should do the brain CT scan and return to him when I have the results. I was also told at my first visit to the doctor that my blood pressure was high and was given two different types of pressure pills which I did not take as my pressure went down days after. I was also give a pill called Plaril 75mg and told that its for circulation cause maybe enough oxygen is not reaching my brain. I am confused and not sure what to do at this point.

I have gone carefully through your query and understand your concerns. It seems that you could be suffering from one of the following:
* vasovagal attacks, possibly related to sick sinus syndrome
* Vasovagal attaks as an effect of fall in blood pressue
* Hypoglycemia or low blood sugar

A single ECG tracing is unlikely to point to the cause. Rather, a 24 hour Holter ECG monitoring needs to be done. The attached blood parameters are fine and not a cause for concern. I would not insist on Plaril 75 right now. Rather, it would be advisable to get a diagnosis first.

Be sure to take plenty of fluids and ORS (Oral rehydration solution) in the meantime.

I would also like to know if my kidneys are functioning properly based on the blood test results since my Creatinine level is out of range (131- increased) and also the estimated glomerular filtration rate was shown to be 59 but when divided as per the instruction (since i am not of XXXXXXX decent) it would be 48. The report also shows that my Hb and rbc to be out of range. Based on the report is my kidney functioning properly?

Your creatinine level is indeed on the higher side and that does indicate some renal compromise. I would need to ask if you are diabetic. I would insist you to get your blood HbA1c level checked. Based on the report further interventions can be planned. If the reports indicate you to be diabetic, the first step would be to put you on strict and regular medication for control of the blood sugar levels. Based on the response it needs to be determined whether you need interventions like dialysis.

I am not Diabetic. The doctor checked my blood sugar level at the first visit and i was told that it was normal.
I browsed through your initial response and now wondering how is it that you said the attached blood parameters are fine and not a cause for concern when the creatinine level, eGF, Hb and RBC readings are shown to be out of range on the attached report. Can you please review the attached blood test results and let me know if I should be concerned with those values. Thanks in advance.

Let me apologize for having ignored to carefully look at the blood report initially. That was because other than a sudden drop in blood glucose (unlikely to get documented in a blood test) no other blood parameter are likely to have any association with the described complaint of blackouts. I should have taken a look but the text of your query did not mention any other problem.

Coming to your question, there is reason to be concerned. A detailed evaluation is needed. The clinical presentation does not somehow match. Even though your blood sugar might be normal, I would strongly insist you to get the HbA1c level checked. Furthermore a thorough clinical evaluation is needed in order to detect any possible presentation of renal compromise.
