                                            Question : Hi,
When we were planning for baby my wife's TSH levels were 3.2. My wife's gynecologist thought that TSH levels are bit high as we were planning for baby hence prescribed Thyronorm 12.5 mcg daily morning. She started taking Thyronorm from the past 1+ month. Now my wife is conceived. The blood work is done yesterday and her TSH levels are tested again. Now her TSH level is 5.53. We are bit worried by seeing this value. Now she is 7 weeks pregnant. We understand that by taking this medicine her TSH level should go down but we are seeing that the TSH level are going high. We understand that TSH level should be very low during pregnancy especially first trimester. We are bit worried is this the right approach? Can she stop Thyronorm 12.5 mcg immediately?

                                    Brief Answer:
She may require higher doses of thyronorm
Detailed Answer:
Hello
I appreciate your concern for your wife
High TSH is suggestive of hypothyroidism, under functioning of thyroid gland.
In my opinion she may require high doses of thyronorm yo be increased gradually upto 25 or 50mcg.
Here I would like further information to help you better
What all symptoms she is having like constipation or weight gain?
If possible upload me all the reports,
Hope this answers your question
Please feel free to ask for follow up with the answers of my questions so that I will be able to tell you more precisely
Best wishes

She is having constipation but no weight gain. She is also taking progesterone tablets Susten 400 mg twice vaginally.
My concern is the approach taken by Dr by prescribing
In spite of normal TSH levels. Now she has to take high dosage and I believe she has to take for long time. Can she stop the medication so that her Tsh levels come down like earlier 3.2 before she started taking thyroid.
Thank you Dr.

                                    Brief Answer:
Please consider these guidelines; she shouldn't stop thyronorm
Detailed Answer:
Hello
Welcome again and thanks for the follow up question
Doctor's watch was ok because he/she has prescribed thyronorm basednon findings of physical examination and complete blood reports
In my opinion your wife has to continue with thyronorm, may be larger doses to meet the demand.
Please upload me the complete report of thyroid function test including T3 and T4 reports.
She shouldn't stop thyronorm which may further increase the TSH level.
progesterone is also required to sustain and maintain in the pregnancy in case of its deficiency during pregnancy.
Hope this answers your question
Thanks for using our services
Wishing her good health and safe motherhood.

                                    Hello Dr,
Thank you for your reply. Her blood report does not show T3 and T4 values. It just shows TSH levels only. We are planning to visit her gynecologist today.
Is there any other way through which she can bring down her TSH levels down. She does not have any family history of thyroid and she dont want to take medicine for long time. Please advise.
Thank You.

                                    Brief Answer:
She should take iodinated food
Detailed Answer:
Hello
Welcome again and thanks for the follow up question
There is no other way to bring it down, she needs to be evaluated in detail to find out the underlying cause for high TSH level.
I would advise her for thyroid function test including T3, T4 and TSH test with assay for anti thyroid antibodies.
However she should take lots of iodinated food like food is fortified with iodine like iodine salt, certain fish and meat products.
Since she is pregnant so you should not stop or add any medicine without consulting her doctor.
Hope this answers your question
Please feel free to discuss further
Best wishes
