Brief Answer:
Paronychia is most probable.

Detailed Answer:
Hi.
Thanks for your query.
Noted of your history of too much pain in left toe on getting up in the morning. Worried about blood clot or bending in sleep.

From the history you have provided this looks to be a case of paronychia, meaning infection in the nail-bed as the pain is too much.

You may need an antibiotic and an anti-inflammatory medicines with painkiller properties like Ibuprofen and most importantly to keep the left lower limb above the heart level in laying down position. This helps in reducing the pain and swelling.
You may please provide clear photographs for on-the-spot diagnosis and any other additional information that you would like to share.

I hope this answers your query, please feel free to ask for further relevant queries if you feel that there is a gap of communication.

Brief Answer:
Not much of edema or redness.

Detailed Answer:
Read and understood that there is a no trauma or blood thinners.
Seen the photo, does not much show any redness or edema.
The pain as you say is at the base of the toe meaning the metatarso-phalangeal joint.

Are the movements of the toe at this joint painful?
Any other associated problems locally or in general?

Brief Answer:
Take medicines and warm salt water soaking.

Detailed Answer:
Blot clotting does not present like this.
So nothing to worry.
Since the pain is only when you push back your toe and the side is bit painful, it is worth waiting fro the results.
I hope you have been put up on anti-inflammatory medicines by your Doctor.

One thing to get a good relief is:
Take a bucket or any container where you can put in your foot at rest.
Take water to fill say 6 inches and go on adding the common table salt till it is getting dissolved to make this supersaturated. Make all the solution warm to be tolerable and put the foot in this warm solution. You will get the maximum effect. Use the same water over and over again. Do this at least twice daily.
