Brief Answer:
Needs further investigation like blood test and ultrasound

Detailed Answer:
Hello
Thanks for posting
I have gone through your query carefully and understand your concern. Let me answer your query one by one-
1. It is normal to have increase in muscle fatigue following an alcoholic drink. This happens because of breakdown of alcohol. This settles of it's own.
2. Loss of appetite and feeling sick after meals could be sign of liver dysfunction along with gastritis. This needs blood test like liver function test (serum bilirubin, SGOT/SGPT).My suggestion would be to divide your daily 3 meals into 6 meals. Prescription only drugs like omeprazole and domperidone helps. Get them prescribed from your local doctor.
3. The on and off pain at lower back is most likely due to muscle spasm and for this Posture correction, hot water fomentation along with muscle relaxant drug thiocolchicoside helps. To rule out any disease of urinary system, Ultrasound of abdomen is highly recommended.
Hope this helps and I shall be glad to answer any further query.
Take care
