What Causes Supraventricular Tachycardia,fatty Liver And Bladder Incontinence?

Question: My health has been deteriorating for the past 10 years. I have supraventricular tachycardia, fatty liver disease, bladder incontinence, severe asthma, allergies, obstructive breathing disease, rashes, itching skin on hands and lower legs, peripheral edema, pedal edema, shortness of breath, chest pain, a collapsed lung recently, degenerative disc disease, chronic bronchitis for 10 winters in a row, orthopedic issues, osteoarthritis in knees, an enlarged right ventricle and was told recently by my doctor that I have liver disease which could progress into cirrhosis and liver failure. I have severe bladder incontinence at night and with all of these symptoms, my doctor checks my GFR or creatinine levels and I am wondering if all the symptoms imply lupus, congestive heart failure or cor pulmonare or kidney failure. My level for creatinine or GFR was 57 and it changes at times from stage 2 to stage 3 and I am worried that my symptoms or health has become so bad already. I am 52 years old and this has become very debilitating. What tests could confirm findings for my medical conditions?

I also have hypothyroidism and am wondering if all of the symptoms are from my thyroid or from kidney failure? Or from congestive heart failure because I have an enlarged right ventricle and I also have an enlarged liver. My abdomen or XXXXXXX has increased and fight it difficult to lose weight. I am also on Liva A Tox and wonder if I don't have a systemtic infection going on?

Sorry to hear about your symptoms.
From the case history, i presume that your Asthma + obstructive airway disease has caused pulmonary hypertension and that has led to enlargement of right ventricle and right ventricular pumping problems. This is caused Cor pulmonale.
When this happens, your venous return from lower half of body is affected and you can have enlarged liver and spleen as well as oedema in legs. Ultimately, you may also have fluid build up in lungs causing lung comgestion which is called congestive heart failure. This can cause shortness of breath and sometimes, choking sensation at night or when you lie down.

You should try to control your lung problems by seeking pulmonology help. Besides, as you have repeated winter bronchitis, it is a good idea to get a yearly flu vaccine and a one single pneumococcal vaccine.

Avoid a salty diet and restrict salt to less than 5 grams a day. Restrict fluid as well. You may also require diuretics to get rid of the excess fluid in body. Try to reduce your weight if you are overweight.

If you smoke, please stop it now. ( did you ever smoke/ passive smooker).

Hypothyroidism also can cause fluid overload and heart failure when it is severe. What is your TSH level like? Are you on medicines?

I am on levothyroxine for my hypothyroidism and also I cough up phlegm and have a long winded cough which makes me gag and get sick at times. I was wondering why the fluid in my system is released so much at nighttime?
Is there anyway I can have the fluid or edema released from my body and keep my body from filling up with fluid?
I just wanted to add that I am on hydrocholorizine for my water retention or edema. Previously I was on Fouresmide, however, it was too strong so I was put a better diuretic. I started to have lung issues after a trip to Mexico to help a group of people build a home. It was very dusty and dirty there and after I arrived back from my week long trip, I had severe lung issues and became very ill. During my time there, I had to enter an old trailer to use the restroom on the site of the home building and the aroma or smell in the air reeked of something toxic and I also froze at night due to the very cold temperatures at night in May.

I also had bronchitis ten winters in a row and now have obstructive breathing disease which makes my asthma worse. Could there also be a bacteria in my lungs or system that also has caused the lung issues?

I also have another question: What is my life expectancy with the lung disease and the build up of fluid in my system? Will my life be shortened or could other complications arise because of these medical conditions?

You have asked me a difficult question now.
Regarding the life expectancy, only god can give a perfect answer. However, one can say that life expectancy is somewhat lesser compared to a healthy individual but with proper maintenance of weight and avoiding excess lung infections, you have a reasonably good life expectancy.

Avoid smoking and passive smoking.
Avoid parks and gardens during pollen season. Avoid pets with fur . Get some one to vacuum clean your carpets and house frequently ( better if you avoid doing it your self). Avoid plants with pollens in your house. Avoid busy public areas and visiting sick people during flu season.

I appreciate the humanitarian work you are doing in Mexico, but dear, your life is important and please look after yourself. Avoid unclean and dusty areas henceforth. Clean air and good water is a must for you.

Your lungs are weak now and is vulnerable to infections. Viral infections are more common than bacterial infections. As I mentioned earlier, you will benefit from regular use of preventive inhalers ( with a spacer) and breathing exercises.
You will also benefit from yearly flu vaccine 1-2 months before the winter/rainy season arrive. You also require a pneumococcal vaccine and repeat it after 5 years.

Regarding diuretic, Hydrocholorothiazide is a very weak diuretic compared to furosemide. It may not really get rid of the extra fluid in your body.
You need to restrict fluid intake and restrict salt intake in your diet.

When you stand up and walk in the day time, the water tend to pool in your lower part of body which will manifest as peripheral edema. This is because your right ventricle pumping is not strong enough to pump back the venous blood which cause pooling in legs. This excess fluid will return to circulation when you lie down at night and this will cause congestion in your lungs and then you end up waking with cough and phlegm. You may be feel better by sleeping with the head end of cot elevated or by using two to three pillows. You may also limit your salt intake ( salt cause fluid retention) and restrict the fluid intake.

Buy a weighing machine at home and maintain your weight. If your weight is more on one day, you may need excess diuretics on that day.
