
What Causes Swelling After Treating Broken Elbow?



My mother, 59 yrs of age had an accident on 1/1/15-elbow broken into two pieces.
Had a surgery (ORIF with 2k wires and TBW-just as shown in the attached picture) on 4/1/15.
Stitch removed on 17/1/15.
Active ROM started the same day and elbow support given.
immediate movement was 120 deg in extension and 90 deg in flexion.
After that she tried her best but no more improvement in movement.
She is not using any support(including the elbow support and the sling) for last 7 days as directed by the surgeon.
But the hand is swelling ( from mid-ulna downwards and notably on the backside of the palm)-there is no redness, no fever, tolerable pain is there.
And the blood test report is like this-
Hb-10, TC-6600, ESR-40, CRP-1.7
My question is-
1) What is the reason of swelling?
2)If ultimately the movement does not improve much-then whether the swelling and pain will at all vanish and then after how many days?

I have studied your case and I think your mother should continue to work on physiotherapy. Her range of movement will only going to get better.
As far as swelling of forearm is concerned it is going to stay for some time (up to 3 to 4 months).It is common after any surgery. It is caused by increase blood flow to that limb,as body tries to send nutrition to healing part. So it is needed for fracture healing.
If sutures are removed and there is no pus discharge from wound than her tests are normal.
Her pain will be vanish in one more month, I would suggest you to give her following drugs.
1.Etoricoxib for pain and swelling.
2. Calcium and vitamin D 3 for better fracture healing.

I would like to restate that she is doing only exercises(active ROM) not physiotherapy.
My mother is trying her best to improve the movement. But in flexion it is at 90 degree she feels very hard to move more.
In the worst case if it does not improve more now, whether latter after the bones join together or after the hardwares removed, is it possible to improve the movement by physiotherapy or by any means?

it is always better to visit a physio because I have seen wonderful result with patients who work under physio.
She should continue exercises and I think she will improve. Her range of movement will improve if implant is removed.
At present we should not put much pressure as some time it can cause fracture non union also. So gentle active exercises, and no force from other person is best for her.

I am back with my mother's problem as i forgot last time to show you the extent of swelling.
I know that you have told, the swelling will going to stay there 2 and 1/2 months more as operation has been done 1 and 1/2 months now.
But to review the extent of swelling, i am attaching pictures of the hand where swelling is maximum(it is the wrist and opposite side of palm).
As the palm is maximally swelled, it has been very tough to do the exercises(closing and opening).
I will request you to see the pictures and suggest.

To recall she has undergone Olecranon ORIF surgery
1)there is stiffness in the elbow movement, but as such nothing in shoulder.
2)There is low intensity pain whole day and night-no such differentiable pain in night.
3)no, there is no such sweating.

If there is only swelling in the hand and there is no stiffness than you should continue physiotherapy. as i told you that swelling is common after any fracture.I would suggest you to arrange daily physiotherapy for your mother. She should also use hot water fomentation.
She can also apply crape bandage at night time.

For the information related to your suspected RSD, i want to mention that
1)There is no such stiffness in the shoulder.
2)expected amount of stiffness in the elbow(the operational site).
3)But, an unexpected amount of swelling and stiffness in the hand(in closing the fingers-pain also).
The last point i forgot to mention earlier.--that's why again i am raising the question.
Will you please reevaluate whether it is suspected RSD?-if yes, then remedies.

If swelling and stiffness is unexpectedly high in hand than we have to think it is RSD. I would suggest you following.
1.Show it to your orthopaedic doctor and confirm the diagnosis. As it is not easy to diagnose it without clinical examination.
2.Daily physiotherapy, hot water fomentation and wax bath.
3.No massage or rubbing.
4.Tablet Amytryptalin 25 mg daily at night time is helpful in such cases. It should be used only under supervision..
If it is RSD than chances of stiffness is High. You need to make sure that she does physio regularly. It take some time to recover.( some time months).

yesterday i met with my doctor here. He suspected it to be "osteodys---?"-the last 5 letters are illegible, he did not clearly pronounced it also.
He suggested hot water fomentation, exercise and voveran 75 sr for medication.
1)What that word can be?
2)Is it the same as RSD?

I am back with my mother's problem-suspected RSD.
To recall, she underwent ORIF of olecranon with TBW. Problem is with the hand portion getting too much stiff.
Recently, i met with the doctor here again. He only advised for hot-water fomentation and the only exercise i.e. to close and open the hand-nothing else.
She is doing the exercise but it is really painful in the hand as well as in the elbow.
She is doing it for 12 days now-without any improvement.
I asked whether wax bath can be helpful-he told that hot water fomentation two times a day and once wax bath are same.

I remember this case. Thanks for using this website. I want to tell you that RSD will take some time to heal. Some time it can be three to five months. It is not possible to see improvement in twelve days. So you should not be worried about it. Soon her movements will improve. Other thing is wax bath is better than hot water fomentation. Your physiotherapist should use it .It has deep penetration of heat than hot water .
I hope she is taking Tablet Amytryptalin tablet in night time. This will give her pain relief and also good night sleep.

Wax bath for six months

She needs to take wax bath for six months twice daily. Physiotherpay should be just range of movement for shoulder, elbow and wrist. Do not go for any massage.
Take Tablet Amytryptalin one daily at night time from 10 mg to 25 mg.

She should do closing and opening of hand. Tretament can not be accelerated as it is a slow process. Any massage can aggravate RSD. So avoid it. If she continue this there is good chance of recovery. Ihope she will be fine in six to

I want to add some more detail about the type of pain she is having.
*** She does not have continuous pain in hand.But when she is doing the "closing and opening exercise"-only then severe pain and it continues for some time after stopping the exercise but no pain when she is in rest.
No such pain in moving wrist,shoulder and elbow.

I have attached the pictures of my mother's hands.The left hand is under consideration.
She takes "voveran sr 75" once daily.
The pictures are of in the morning, when swelling is minimum, though it increases slightly later once she does the "closing and opening of hand" exercises.
further, she is feeling slightly "trigger finger" condition in the middle finger.
When in rest, no such pain but when doing specifically the "closing and opening of hand" exercise, the pain is extreme and continues for 20-30 mins more afterwords.
Please asses the condition and suggest.

I have studied her images. She is having RSD. As in images you can see that her skin is shiny, loss of creases and swelling is also present. So It confirm our diagnosis. As you are saiyng that she is not having pain, it is because she is taking pain killer daily. I have following suggestions for you.
1.Steroid injection for trigger finger.
2.Wax bath and physiotherapy.
3.Tablet Etoricoxib 90 mg daily.( safe than voveron )
3.Tablet Amytryptalin 10 mg night time if she is having pain in night.

I am always enriched by your suggestions. But problem is that i can not go for steroid injection straightway without consulting the local doctor.
I met with him here.
But he now suggested for something else-"IFT/NV stimulation" for the hand for ten days once everyday.
I am doubtful whether this will anyway affect the operated olecranon or not as it is two and half months now the operation has been done.
please suggest what should i do.

If you want than you can discussed about steroid injection. Though there is no harm in local injection. It should not be given in blood or in form of tablets.
IFT/NV stimulation is not going to harm olacranon fracture. So you can continue with this.

here i have gone to a physiotherapy center. They are giving the stimulation and they are calling it "Faradic stimulation".
1)whether it is the right stimulation they are giving?
2)Whether it is right to take stimulations i.e if this does not work whether it will create any side effect?

Here i continue with my mother's condition.
Expecting that you can recall the case, i would like to give the update only:
1) the elbow movement is now very good-complete in flexion but 15 deg short in extension.no problem in wrist and shoulder movement
2)I understand that in fisting the hand there will be pain in the hand but when she tries to make the fist, pain(pin pointed)happens to start in the operational site(i.e elbow region)-this pain refrains her from making the fist.
I would like to get your comment on the above.
Should i consult with a nuro-doctor here? whether any nerve is getting disturbed by the hardware in the operational site?

It is good that her movements are improving. She should continue with physiotherapy. I hope she is taking Pregabalin and Meghylcobal as I advised before. Some time nerves get stretched during physiotherapy. So that type of pain can occur some time, but it is nothing to be worried about and there is no need to show it to neurologist.
if she is not taking Pregabalin and Methylcobal than she can take one tablet at night time. This tablet causes giddiness so should be taken just before sleeping. I hope this answer will be helpful for you. Let me know if there is any other follow up questions.

Please note that:
1) with hot water fomentation or wax bath, she can make the fist but within 5 minutes the condition is reset.
2) every time when she tries(whether she can make it or not) to make the fist, the pain starts at elbow's operational site.
3)Further in the hand, though it is swelled, pain is not that much.
Please review the points and give direction.

2.Pain at elbow due to wrist movement is due to stretching. It is nothing to be worried about. I do not think it is any serious issue.
3.It is good that pain is decreased in hand. Soon swelling will also go. It may take few months though.
