Brief Answer:
Urgent admission and surgical treatment needed

Detailed Answer:
Hi, thanks for posting the concern in HCM. I can see that you have asked the question before and have been answered very efficiently by Dr. Chandrakant.

Now, I like to add a few thing.
Firstly, the condition is really dangerous and needs urgent surgical attention. Simply speaking, the medicine you took tried to engorge your penile vein and while doing intercourse, there may have been some injury due to extra effort and the result is local congestion, stoppage of supply, stasis of blood with surrounding edema and necrotic process has started setting in. If blood supply is not restored immediately, their will be permanent damage and death of the local tissue resulting in auto-amputation of the penis alongwith systemic complications like dyselectrolytemia, sepsis etc.
So, you can now understand the gravity of the situation.
Now, the other face of the coin is that, if you are managed early by surgical intervention, you will get miraculous result.
Management plan will be-
Relevant investigations (efficiently mentioned by Dr. Chandrakant previously)
Good parenteral antibiotic
Oral analgesic and anti-inflammatory (Tab Paracetamol-Aceclofenac combination)
If penis tissue is still viable- release of local pressure and congestion
If tissue is non-viable- Amputation
Now, the thing is that, whatever his current status is, you MUST TAKE HIM TO A LOCAL SURGEON OR HOSPITAL AND GET THE TREATMENT DONE.
