Question: Hi,
My 3year old daughter has developed the allergy to the paracetamol. Earlier it was not the case and since last 6 months whenever i give her paracetamol during fever, her lips and only left under eye gets swelled and pinkish. It later turns to dark and chopped slightly. Any help on this is appreciated. Please help.

I have gone through your query and understand your concerns. I have checked the attached images and do not think that it is due to allergy to paracetamol. Rather, it seems that your child has a viral infection. Such infections are often associated with rashes. Ideally a closer and direct examination would have been invaluable but that is beyond the scope. You can give her some antiallergic like cetirizine once daily for the next few days. You may also apply some emollients or moisturizers like aloe vera lotion topically.

You may get back to me in 3-5 days in order to provide updates. Hoping for the condition to improve by then.

Thank you so much for your initial suggestions. The redness appears only after we put her paracetamol and never seen before while she was normal/during fevers. Once the paracetamol goes in her body, after 2-3 hours the redness starts to appear. When it happened last time we did applied the emollients or moisturizers and cetirizine in the past and it has no benefit healing the cause, instead it made the wound worst and the local doc suggested to stop everything and suggested to massage with ice cubes. So we are leaving it as it is and just rubbing with ice cubes that has a greater help in healing her. At night we are applying coconut oil to keep it moisture.

I am really worried how we could control this or make not happen again. Really appreciate your further review.

Thanks for clarifying. In such a case, you may try not giving her paracetamol. Alternatively you can give her Ibugesic syr (ibuprofen) or Meftal P (mefenamic acid) in case the temperature rises. As of now, follow the steps suggested in my earlier communication. Coconut oil or any other moisturizers should suffice. Ice can be applied.

If you have correctly identified the trigger to be paracetamol, simply abstaining from its use should lead to resolution in 3-7 days. I would still insist on giving some antiallergic like cetirizine.

You need not worry so much.

Hi Sir,

Thank you so much for the alternative suggestion. It would be great help if you can share me across the suggested medicine images because when i searched, all i can see most as Ibugesic plus and there are different mg. So to be clear can you please share me the exact medicine images and dosage. She is 10kg in weight and looks skinny.

I have also attached the my search results. Please review the mgs and suggest which one is preferred as low dosage.

Thanks for writing back.

Ibugesic Plus contains paracetamol along. Rather plain Ibugesic should be given. For a 10 kg child 5 ml of ibugesic is right to be given.
Meftal P (100mg/5ml) 3 ml is to be given to your child. If it is 50mg/5ml, double the amount.
