Question : My scrotum sack is swollen and tight to my body, also get pain in my back that kinda wraps around from my lower shoulder to my left hip

Brief Answer: Scrotal swelling may be due to many reasons. Detailed Answer: Hello XXXX Thanks for writing to us For scrotum swelling you need proper clinical examination and investigations. It may be due to many reasons.It may be due to hernia,hydrocele,epididymitis,orchitis,varicocele,testicular tumour etc. Testis may or may not be involved.It is difficult to say without clinical examination and investigations. You need routine investigations like routine hemogram,RBS,RFT,urine RE/ME. Investigations like Ultrasound of scrotum and Doppler should be done.It is important to know history of fever,generalized edema and pain. Ultrasound of abdomen and CT Scan scan be done if needed.You should consult surgeon. Back pain may also be due to many reasons.Probably it is due to muscle spasm as you do lot of manual work.You can take analgesics like tablet Acelofenac 200 mg twice daily for five days along with muscle relaxants like tablet Tizanidine 4 mg twice daily for 4-5 days.Besides this you should take rest and hot compression.It will relieve you from pain. X-ray of Dorso-lumbar spine in AP/Lateral view should be taken. If pain is persistent you should consult orthopedician. Get well soon. Hope i have answered your query. Thanks Dr. I am adopted so not much there. But I did find out as a very young baby I had some type of hernia. I have regular bowel movments and urinate regularly both pain free, no blood in either. The pain in my shoulder that goes around my side to my hip hurts. When I first do anything physical it can bring me to my knees then relents. I have had some swelling in my right toes, so I thought with a constantly tight scrotum and the edima, plus both legs on my calves has bumps that are dry and itch. This all happened about a week ago. I went to ed like normal was awoken with very short breath, high fever, bad sweating. After a half hour I felt like I had a bad flu and took motrin and went back to sleep. A week later the scrotum swelled. Has never gotten bigger. If I apply heat I can make it shrink but it does return. Can this be an infection or bloord flow related? I have always had a family dr, a year ago she was diagnosed with cancer and stopped practicing. I have insurance but not great can not just go to ER. I need some help as to whom to see and can I see without a primary referal? Thanks I do have access to medication if taking that might help. Doycycline was given to me by a friend, but I do not want to just take stuff, but the cost of going to a bunch of dr's I can not afford. I just wonder about the bumps on my legs and the scrotum is tight which I think means the testicles are cold. So the fluid around them is colder causing the scrotum to pull tight like there is not enough blood to keep them warm, just guessing and based on how I feel. Is the muscle pain in my back and sides and it takes my breath away related. It started after that night when I was awaken from sleep. I have no chest pain and my heart seems fine not a problem from what I can feel. I have very little $$$ so I want to do the right thing but am so lost as to where and whom to start. In a weird way other than it feels funny and uncomfortable walking it does not bother me. The pain wraping around my side can be reproved with heat but never goes away

For back pain you can take medicines as discussed earlier. You shouldn't apply heat to your scrotum.It can cause you permanent damage. Thank you for your help. Applying heat to my pelvis were my leg meets my hip is probably a better description. I have put the partial end of the heating pad on the sack. Why is that bad? What does it do? I am still curious about the flow of blood and can the edima of the foot and the swollen scrotum be tied togeather? They all started after that night when I woke up in a cold sweat andva fever. Do you think it is possible or is it your general opinion without seeing it a hernia of some sorts? Is the back pain tied into it, is that type of back pain associated in general with hernias? What type surgeon should I see? I understand that getting a possible ultrasound I can get an exact diagnosis, but $$$ is a huge factor. When I go I have or want to go to the right type of dr. not passed from one to another, I will run out of money and never get fixed. Please give me your opinion and point me in the right direction. Thank you What type of dr or survey should I go see. Do I need to see a generalnpractioner or can I skip that step as it will cost me additional dollars. Everything started that night I was woken up from my sleep. Even that day I went to work worked all day went to watch my sons basketball game , ate diner. A normal night. It was not until after I was sleeping did the problem start. It has not happened again. I have not had any type of attack or episode since that night. One and done. Just using common sense that had to be when it happened, it may have been present before that night. Why the bumps on my calves? Also from that night. It took about a week after that night for the scrotum to swell and the itchy legs and sore wrap around back kinda pain. Again, not to keep belaboring the point. I need and want it fixed but can not afford to go to the wrong dr. Again thank you

Thanks again. A general surgeon , not a urologist ? I take it you do not feel it is a blood flow problem? I will call a general surgeon on Monday . Today is Saturday , is that to long to wait. I noticed the last 24 hrs the sack is reduced in size. Not half of the size it was , I would say when it was fully swollen looked like one side had a golf ball size while the other was the size of a medium gum ball. Why has it shrunk some? I have stopped using heat since I read your post. I never used to wear any type of underwear, ever, now I wear underwear all the time except at night when I am rating or sleeping. I was told wearing underwear or better yet an athletic supporter can make it feel better and help the problem, is that true? Again thank you for your help
