What Causes Swelling In The Ankles While On Lisinopril-hctz?

Question : I am on lisinopril-hctz tabs 20-25 mg and have had no problem over last 15 years. I noticed just recently I was having slight swelling in ankles and so I got ou t be monitor and it reads 161/101. I intend to get appt. with Doctor tomorrow since it is Sunday. Is this more emergent? I am an extremely healthy and active 80 yrs. old

Regarding your concern, I would explain that your symptoms (leg swelling) could be related to fluid retention. The main causes of fluid retention are heart failure, liver or kidney disease. Other possible causes to consider would be chronic venous insufficiency and diabetes.

Besides your blood pressure values are a little high, which means that your blood pressure is not under control and you may need to make some changes to your antihypertensive therapy if these blood pressure values persist.

But, no one of the above mentioned disorders is emergent. So, the situation can wait until consulting with your doctor.

It is also important performing some tests, besides a careful physical exam:

- a resting ECG and cardiac ultrasound to examine your heart function and structure
- NT-proBNP levels for heart failure
- a chest X ray study
- kidney and liver function tests
- blood electrolytes
- fasting glucose
- an urine analysis.

A Doppler ultrasound of the leg vessels would be necessary if suspicions of chronic venous insufficiency are raised during physical exam.

You should discuss with your doctor on the above issues.
