default
Posted on Mon, 10 Apr 2017
Question: Hi ,
Good Evening.
14 days back i had some swelling at groin area. When i went through US scrotum&inuinal ultrasound scanning the results were below
Few enlarged inguinal nodes on right side, largest measuring 17 by 7 mm with adjecent.
Soft tissue inflamation.

14 days have been passed and now the swelling is reduced to 90% than earlier. But some times i sense some flucuations in that area. But its reducing.
Unfortunately from yesterday i am experiencing little pain in testicles like when it touches to my thigh area or some other body.I am using cepodium 200 from the past 14 days. Please advice how to proceed further. Is it a kind of infection.I too noticed some swelling at armpit.

Brief Answer:
Clinical evaluation, investigations and proper treatment

Detailed Answer:
Hi.
Thanks for your query.
Noted the history well, seen the reports you posted and understood your concerns:
The inguinal nodes must be of infective origin and the source depends upon the presentation.
Thus if the vertical chain of inguinal lymph nodes is affected, the source is usually in the lower limb. If the horizontal chain is involved the most probable cause in skin of the scrotum, penis and peri-anal area. The source can be a trivial one and hence not always noted or found out. Sometimes the cause can be a general infection of the whole body and many a lymph nodes can be involved.
Since you have good response with Cepodem, the cause was most probably infection.
The pain in the testicles is usually not associated or related to the inguinal lymph node swelling or infection. Can be due to mild infection and your urine report shows 2-3 pus cells although report is old - I think it may be of 1 March 2017.
Hence it is better to repeat the urine and blood reports.
Also go for the Ultrasound again of the scrotum and of the armpit to know the cause and all these tests will help in diagnosis. FNAC should be done as it helps in better diagnostic acumen.
As also is very important the clinical evaluation and examination by your Doctor, preferably a General Surgeon to get a proper diagnosis, get started/continue with proper medication, further investigations as may be necessary and to follow up properly till cure.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.

Thanks for the information.
In the report it was written that right inigual nodes enlarged, horizontal group.
I had intermittent fever for copule of days, during that time my body sweated a lot and had extreme fitigue and from yesterday i didnt experience any fever or chills.
I forgot to mention one more thing in my earlier communication.
I sense little swell on both shoulders ,i feel this when i turn my head on either sides.
Is this all relates to certain kind of disease or its too early to decide upon it.
As suggested i will have all the tests done in a couple of days.

Brief Answer:
Awaiting for the reports.

Detailed Answer:
Thanks for your nice feedback.

Your history gives the description regarding the things we discussed above.
Swelling on shoulder- very rare to know, not sure about the cause.

To decide the kind of disease, it is better to know the reports, proper clinical evaluation, any further investigations as medical science is based on fact to get the most proper diagnosis in order to provide the most proper management.

Awaiting for those reports.

Brief Answer:
As detailed below.

Detailed Answer:
Hi.
Thanks for your feedback and reports.
Axilla has folliculitis - the infection in it might have caused the two lymph nodes to get enlarged.
No enlargement of liver, spleen or lymph nodes in abdomen - a good thing.
Small cyst in left epididymis is not significant. The left inguinal nodes have normal central fatty hilum hence taken as not-significant. They are discrete, meaning separate from each other - not matted together.

I hope you are under treatment as you have consulted a General Surgeon.
Insist rather request for FNAC at least by discussion with your Sonologist/ General Surgeon. Helps in ruling out any serious problem if the lymph nodes do not show regression after a complete course of treatment.

I hope this answer helps you.
