What Causes Swelling Of Skin?

Question: hi, can you please tell me what you think this is? it does not itch, it is not bothersome at all. it is only shiny or wet looking because I just put neosporin on it, I don't know why I put neosporin on it, I have no reason. this is on my stomach the under part, I am very over weight, obese, its like underneath the apron area. please advise.

- Is there any itching burning or any pain. If yes then is it mild/ moderate/sever.

- Are you taking any medications for the same or have you visited a dermatologist or a doctor for this condition.

- Any other medical issue or illness or medications.

I have had this a long time, months, many months. it does not bother me at all. In fact I wouldn't even know it was there unless I saw it. I am on pain medication for a hurt back & hip. I take oxycodone & morphine. I was seen at the ER a couple/few weeks ago and all my blood levd tested fine. my liver is functions correctly. they also did some heart test ekg maybe & that was fine too. I is also on or near my knee & thigh area where that pad of fat is... I am very large and have "extra" fat there. I think it is obesity elephantitis or lymphedema. I am going to try and attach more pics

Hello,

Welcome again and thanks for posting your important details to the forum.

After analyzing the complete situation and pictures I can make out that the condition of skin is "Peau d'orange" with probably lymphedema".

It is a condition where there is swelling if the skin and resulting in appearance of skin just like orange skin. Peau d'orange is caused by cutaneous lymphatic edema, which causes swelling.

There can be multiple of the causes of the same and we require to go for a set of investigations as well as physical examination to rule out the exact cause.

For the same I suggest you to meet a surgeon near you or best would be at Medical college level near your place. At medical college level a multidisciplinary approach could be done to help you best.

To take care of the problem temporarily kindly follow the suggestions below:

1. The most important measure is to take care of the swelling. You should wear those kind of clothes by which there should be support to the swelling. If unsupported then the swelling can increase day be day. Do not pinch, pop or squeeze or even touch any of the affected areas. I suggest you to even not to touch the area unnecessarily.

2. Do not apply any creamy or oily stuff with fragrance in it directly at or near the affected areas.

3. Try to avoid any home remedies suggested by your friends or family members or otherwise self implicated therapies also.

4. Try avoiding situations of heat, sweating and humidity for long hours. Do not take hot showers and avoid heavy exercise and gym for few days.

5. Do compressions with cold water for 3-4 times a day . Do compresses by taking a towel dipped in cold water for 5 minutes each time. It will not only soothe skin but also will reduce the swelling.

For the part of immediate treatment in my patients with your kind of problem I suggest following measure:

1. I suggest my patients to apply "Fluticasone" cream over entire face in a very thin layer for two times (morning and night) under medical guidance. Apply it in such a thin layer that it should not be visible by naked eye. Do not rub it. Do not use beyond 7 days without dermatological advice. If Fluticasone is not available then alternatively hydrocortisone or mometasone cream can be used. It will only help in reducing the redness as a temporary part. It is not the actual treatment.


2. For reducing the itching or any irritation I suggest to take tablet "desloaratadine" (Claritin) by mouth under medical guidance for one time if is less or for two to three time if the irritation is more.

just a few more questions. why would I see a surgeon? may I continue to use Hibiclens soap? what exactly is the treatment for peau d'orange AND what is the treatment for cutaneous lymphatic edema? how exactly are these two things diagnosed, exactly what testing should be done?
I am actually leaving for vacation in about 6 hours for a week and half. I already have a Dr appt scheduled for when I return about something else but now I guess my dr & I will be discussing this instead. I will do the cold compressions to keep the swelling down.

As per the situation the swelling appears to be quite big. In such a cases surgeon are the main person to analyze the condition and then guide you accordingly. A surgeon's reference not only rules out any internal cause of the problem but will give us a pathway for further treatment and management.

There are so many causes of peau d'orange and lymphedema and the treatment is suggested accordingly. Peau d'orange and lymphedema are just symptoms or expression of the internal thing and not a disease entity itself.

For the diagnosis of same a complete blood examination with liver profile, renal profile and an ultrasonography are required. A further specialized investigation of MRI or CT scan can be done according to the findings of the reports.

You can continue the Hibiclens soap. Do not stand for long hours and let the swelling be supported by proper clothing.

Mild obesity is not a factor. High obesity can be one of the important cause of both the problem because of tense skin and more of fat accumulation. This can lead to impaired blood as well as lymphatic circulation. You can discuss this issue with the doctor if you have high obesity.

A rule of thumb is that a person of 5 feet should have weight of 50 kg and 6 feet around 60-70 kg. So accordingly you can judge your weight status.
