Question : Hi doctor,
I had a light sex with my gf a couple of days back. I did not use a condom but did not insert in the vagina. I was stroking from the back. Tried to insert but since I was an ameateur I ended up hurting myelf. After that I had her give me some hand job which was quite hard. When I saw my penis after that I saw a small bulge or swelling at the back of the penis head which looks wierd. The skin was down conplety and was not going uP because of that bulge. There is no pain and urination is normal. It's been 2 days now ans the skin isnt completeky up yet. The vulge at the back jas reduced a lil but when it erects witj whatever reason during the course of the day it becomes a lil big again and the small later on. Thats preventing the skin to go up completely. I am worried. please advice

I have gone through your query. Your problem can be most like paraphimosis. This occurs if foreskin left retracted for long time. Ice application will be helpful to relieve the swelling. Lubricants can be applied also to retract foreskin back.

If the swelling is not not reducing and foreskin not retracting back then approach a general surgeon or ER as it is a medical emergency. If you keep the fore skin retracted for long time then it won't be viable and circumcision should be done.

Thanks for your response Dr XXXXXXX Well, I have some concerns and would like to know your honest suggestions to them.
1. What could be the cause of this? I never had such a problem before even though I indulged in light sex.
2. The foreskin is slowly going up. How many more days do you think I should give it before getting worried?
3. What could be the reason of that swelling at the back of penis head? Is it because of that the foreskin can't go up completely?
4. If I go for non surgical methods what could they be? What is the cons of having the foreskin not completely covering the penis? I'm not too sure so just looking for an honest opinion from you.
5. If there is no pain should I still be worried?
6. The penis head since it doesn't have the foreskin completely over it when touches any garment gives a weird feeling. I mean its not pain just that sensitive feeling. How can I get over that?

Thank you for your follow up.

1. The reason can be due to injury to foreskin during sex or due to unretracted for long time.
2.If foreskin is completely retracted then it can cut its blood supply and cause non viability for foreskin. In such case it won't be reversible and circumcision is the only remedy. Within 1 day it should return back. If not retracting back you should approach your doctor for manual reduction.
3.Yes back looks swollen due to non retractable foreskin.
4.Non surgical measures are:
-using ice packs,
-compressive dressings,
-With the help of local anesthetic retract manually,
-applying mannitol soaked gauze above the edematous area and cover up with a condom or glove.
-Injecting hyaluronidase over several sites which helps in decrease in swelling.
The main problem of unretractable foreskin is risk of gangrene (non viability) of foreskin.
5. If absolutely no pain is there you should be concerned as it can be a sign of absence of blood supply.
6. That sensitivity of place where foreskin is not covered is normal. Even if circumcision is done such sensitivity will be there for some time. It will go gradually.

What you can do at home is cover penis with a towel and apply ice packs to relieve swelling. If the swelling is decreasing and foreskin is getting reduced then ok. If not reducing then better to approach your doctor early for manual reduction with local anesthetic.
