Posted on Mon, 5 May 2014
Question: My perineum is swollen and painful. Could the result be from my first time engaging in anal sex with my partner? However, the last intercourse was over a week ago, but the area still feels swollen and painful. And my anus is also sore and I experiencing some constipation and pain when passing stools. I have been using hemorrdial cream, hydrocortizone cream, and olive olive to try and relieve the symptoms.

Brief Answer: Yes this can happen with anal intercourse Detailed Answer: Dear Friend. Hi , I am Dr Anshul Varshney , I have read your query in detail , I understand your concern. Yes this can happen with anal sex especially if done for the first time. It can result due to following reasons: 1. Friction during anal sex leading to local swelling due to inflammation 2. Infection due to unprotected anal course. 3. Allergy to latex (if condom was used) Also the pain and soreness in anus seems to be due to that only due to stretching of anal sphincter. I would advise you to simply use a local antibiotic cream like T bact over the affected area if it seems red and if any evidence of infection is there and to apply a moisturizing lotion there. Don't use hemorrdial or hydrocortisone cream. Take high fibre diet in order to avoid constipation that can further aggravate your problem This would get relive in a few days. You may also do cold fomentation by ice packs for symptomatic relief. This is my personal opinion based on details available here. If you still have any query you may please ask me. Stay Healthy. Dear Dr. Varshney, Thank you for your prompt reply to my questions. I am an American, but I live in XXXXXXX so access to typical medical products can sometimes be tricky. Is there a generic kind of antibiotic cream that I can ask for at the pharmacy here? Also, do I need to be worried about a STI because the anal sex was unprotected? However, my partner gets tested every three months and hasn't had any problems with STIs in the past.... Or, for instance, would Neosporin work as well? (That I do have here in medicine cabinet, brought from the States).

Brief Answer: Detail Follows. Detailed Answer: Dear Friend T Bact contains Mupirocin. Even if you are unable to get it, you can wait. Just apply moisturizing cream over that area. You may also take a local anti inflammatory pain killer like Tab Zynase-D (Diclofenac Potassium, Serratiopeptidase) Twice a day for 3 days. About Sexually Transmitted Diseases, i must say you are always at risk if you play unsafe and any of the partner has multiple sexual partners. So, i would say that you should go for protected sex, that is the best way to prevent STD's. If you have any further query, please ask m.
