Question: I have been seeing a Rheumatologist for about 10 years now. I have been on Methotrexate all that time and have been on prednisone 5 mg for about 6 years. In December I went on a trip to New York for 1 day and was on my feet for about 10 - 12 hours. The next day my ankles swelled and stayed that way for about 3 weeks. Then my hands starting hurting terribly and knots came up overnight on them and they stay red. I also started having a lot more pain all over my whole body. They started my on Imuran 50 mgs and told me to stay on the Methotrexate with the prednisone. By blood work is always normal my sed rate stayed 0 for a couple of years but went to 3 during this flare or whatever it was. For years I was told I had seronegative RA now all of the sudden they are telling me they don't know what is wrong with me but sending in a SLE diagnosis to the insurance company. Also, they are taking me off of pain medication which is fine but I want to know what is wrong with me. My spine is in terrible shape also even in the lumbar spine it says along with facet arthopathy I have modic end plate changes. My hands look just like a picture of Jaccaud's arthropathy which I found while trying to figure out what is wrong with me. I feel like because my labs are all normal that they thing nothing is wrong with me. I would like your opinion and also your opinion if I should go to a large arthritis center such as XXXXXXX Clinic or John's Hopkins?

Brief Answer:
I would need to see your autoimmune profile.

Detailed Answer:
Dear Sir,

Thank you for posting your query on HCM.

Well looking into your query I will like to tell you that I would like to see your autoimmune profile like ANA , ENA etc along with RF , Anti CCP antibodies . Sometimes rheumatoid arthritis and SLE overlap so there is a possibilty of one changing to another but before seeing the result cant comment on it . Also did they check for sarcoidosis also as what you tell ankle swelling and knots sound like erythema nodosum . Please send the blood work .

Take care.

Dr. Shruti
