Brief Answer:
Active infection

Detailed Answer:
Hi,

Thanks for your query.

Swollen ear, nausea and vertigo are symptoms seen in active middle ear infection.

If you are on Doxycycline for the ear infection, I believe there is a need to change the same. Doxycycline is not suitable antibiotic for middle ear infections.

Fluoroquinolone class of antibiotics are best suited for ear infections. I suggest you to ask your treating doctor to consider the same.

Hope, I answered your query.

Wish you good health.
