What Causes Swollen Tonsil, Sweating, Weakness And Fever?

Question: Hi,

I have been sick since last saturday with the following sumptoms:
1) fever 38 taking tylenol doesnt help
2) left tonsil severly swollen red and white patches
3) extreme sweating
4) weakness

Tests:
Blood tests singificant information:
On saturday i had a WBC Leucocytes amount of 5.2 (range 4.5-11)
Today (thursday) same
Test gave me 4.4
How is it possible my white blood
Cells are decreasing

Also tested ALT reference range 0-30 my result is 47. Doesnt this indicate liver damage?

Had test of EbV antibodies- still waiting for results.
Throat culture negative for strep A
Urine culture came back with mixed
Bacteria

Medication:
Dr originally prescribed pen-rafa. Took it for 3 days- did mothing so was changed to co-azithromycin (6 pill pack)
Tylenol every 4 hours and vitamin b and c

What is going on? Is this mono or sonething else. Why are my wbc numbers decreasinh an my alt increasing.

I am 33 yr old female.

Dear Friend.
Hi , I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.

This seems to be Follicular/Bacterial Tonsillitis/Pharyngitis

Azithromycin won't help in this

Urine sample can be contaminated.

WBC count can decrease as a result of Viral Infection.

Irrespective of test results, i advise following in my patients:

1. Antibiotic Augmentin 625 mg Thrice a day.
2. Antihistaminics like Levocetrizine 5mg once a day\
3. Warm Saline Gargles
4. Paracetamol on as when required basis for fever.

If you come out to be positive for Mono, add doxycycline as well.

This usually takes 3-5 days after you get on antibiotics.

ALT can raise if you are consuming excessive Paracetamol.

If it gets further high, you would require further investigations
I would advise you to discuss with your doctor for starting you on these medications.
I am sure you will feel the difference with in 24-48 hours.

So bacteria in urine is not surprising. Should i continue the azithromycin for this?

What are normal blood and physical symtpoms of mono?

Dear Friend.
I would advise you to get a Urine Culture done.

Actually, when we get once source of infection like Tonsillitis, we don't suspect unnecessarily other sources of infection.
But since you had an episode of Urine infection earlier also, i would advise you to get Urine Culture and also Ultrasound Abdomen with KUB.

Azithromycin won't help if you have severe Bacterial/ Follicular Tonsillitis.

Infectious Mononucleosis usually presents with fever, Sore throat, Lymphadenopathy.

This is a self limiting condition, that is why i am not stressing on it.
