What Causes Tension Type Headache?

Question: Hello Doctor, I am a student (24 yrs) I have been taking betacap tr 40mg, tryptomer 10mg and feliz s 5mg for 2 months for cluster migraine headache with TTH and anxiety.Ever since having my new medications , my headaches have increased and have been suffering from dizziness,abnormal vision and lot of pain in my right eye.What does TTH mean and what should I do to get better or get my medication changed?

Hello XXXX,
I have gone through your question and understand your concerns.
You have been diagnosed as migraine with tension type headache(TTH) and is being treated accordingly.
The treatment of migraine requires life style modifications (adequate sleep, timely food, avoiding chocolate, coffee and other aggravating factors)
Drugs for prophylaxis are many and can be used in combination too.
If you are not improving on these drugs then you can discuss regarding change in medications.
I usually prefer Flunarizine and amitriptyline as first line drug and other drug like divalproate or topiramate later on.
For Tension and anxiety Sertraline can be used.
Eye pain is not a feature of migraine and you may require reassessment if there is unilateral eye pain, redness, watering and nasal stuffiness for possibility of cluster headache or Short-lasting unilateral neuralgiform headache with conjunctival injection and tearing (SUNCT).
The duration of headache associated features and aggravating factors are important part of history.
If there has been recent change in medications then you may wait for around two weeks for the full benefit of drugs.
Pain killers may be taken on sos basis if required.
Life style modification is the key.

Naproxen or Aceclofenac can be used for acute pain relief.
Take with omeprazole or pantoprazole to avoid peptic ulcers.
