What Causes Testicle Pain During Erection?

Question: yes,when I start getting a erection my right testicle starts upwards and I have severe pain.sometimes it stays pulled up as far into my abdomen or into my leg as it can.I have had this problem for several years been to many doctors with really no answers.at times i cant work for weeks at a time i hurt so bad.I have been diagnosed with chronic epidydimitus. I have a large mass of something growing off the bottom of my right testicle about half the size of a golf ball.i hurt up into my stomach my cords are very large and hard.my right testicle stays swelled and hard with a thick feeling something growing on it.even if i have sex and ejaculate it still hurts.as soon as the erection starts the pain starts in the cord where it attaches to the testicle.it hurts so bad it stops sex dead.i have to have some relief i cant take this anymore

Thanks for your query and an elucidate history.
To recapitulate: Male/52 - diagnosed chronic epididymitis with cyst - large mass - half the golf ball - at bottom of rt testicle - swollen and hard - pulled up with increase in pain on erection - stops sex - stays pulled up - hard and large cord - several years - many Doctors - no answers...
As per the history you have provided, this looks to be right sided epididymo-orchitiis with spread of infection/inflammation to the cord as all these structures are in continuity.
During the excitation the lower abdominal muscles and their extension called cremaster muscle goes into spasm as would normally done but very strongly as seen in your case as it must have been inflamed and while doing this it causes rotation and retraction causing the pains that you have explained.
I would advise you the following:
Get /post the recent ultrasonography and color doppler of scrotum and abdomen, if not done recently , get a fresh one done and post the reports.
Give some additional information:
- Are you married? If yes, children?
- Any consultation with an Urologist or General Surgeon? Anyone advising surgery or so?

Also get the following things done:
Urine : routine, microscopy, culture and sensitivity.
Tests of blood: CBC, Kidney functions, alpha-fetoprotein (AFP), beta human chorionic gonadotropin (ß-HCG) and lactate dehydrogenase (LDH).
MRI of the scrotum if done gives the best picture.
Semen analysis is helpful.

All these will lead to a proper diagnosis and give the best plan of treatment which depends upon many factors as mentioned above.

As much as thats not what I wanted to hear thank you.I have seen 3 urologist, been to er twice and laid in pain more than I care to think about.Out of all of those I got a better answer from you than any of them.I have had 3 ultrasounds and been told there is nothing that can be done for you to you just need to pee and your interpreting that as pain.I have asked to have the testicle removed and nothing.I cant hold a public job because of it.When it hurts which is often I can only lay in floor or tub of hot water and scream.Its pretty much a daily thing at some point in the day.I am divorced but living with a girl,one son age 24.I have had other test done I don't remember what all.I have read everything I can find about it.There has to be something that can be done to control the pain.I also have ED but had found some over the counter pills that worked until the FDA banned them a few weeks ago.Now I'm left with a 28 year old GF that I'msure won't be around long if things don't work.where can I go and what can I do to help myself? what do I need to do?

I can understand the plight you must be going through.
I would advise you the following:
Have another opinion of a XXXXXXX General Surgeon or another Urologist. The precious Consultants might be worries about the intrinsic issue connected with your problems.
Insist for MRI of the affected area as it gives far more information than ultrasound and color doppler.
Scrotal support.
Warm water bath as you are doing.
Avoid anything that contracts the lower abdominal muscles too tightly.

Send me the report of MRI and give feedback about what other Specialists think about it.
