You should rule out any kidney or liver disease if present by following investigations.
_serum sgpt, serum sgot
_serum urea, serum creatinine
_egfr estimation
In liver and kidney disease detoxification process of body affected. So ammonia not detoxified and smell can be there.
If your liver and kidney profile results within range then rule out chronic sinusitis and Rhinitis by paranasal sinus X Ray and rhinoscopy nose examination. For nose examination through rhinoscopy, you can consult otorhinolaryngologist.
If all these comes normal then no need to worry.
