What Causes Tightness And Pain In Neck Along With Headache?

Question: I have large goiter that suddenly seems tight in neck,slight pain and have headache.This happened two days ago,also there looks like a dark maroon protrusion size of a dime in middle of goiter.No problem with swallowing.Had this pain a few years ago when goiter was smaller.Amoxicillin and excedrin helped in the past,does not seem to be helping now.My Doctor is on vacation for two weeks ,what do you suggest for the pain?

Sorry to note the pain.

When I see someone like you in my practice I offer them Tylenol or non-NSAID type pain killers (aspirin, ibuprofen are examples of NSAIDs) but only for a day or two till they come see me personally. The reason for this is that the NSAIDs may cause further bleeding if the is already some bleeding inside the thyroid.It seems unlikely but there is a possibility of this based on what you are describing, especially the pain and the maroon patch.

Moreover, this requires prompt medical attention. I understand your doctor is out of town for 2 weeks but it would be best to see an endocrinologist in-person immediately to resolve the matter.

The symptoms you are reporting need to be investigated, and a thorough physical examination is in order too.

When someone presents to me in my office with a goitre and complaint like yours, I order the following tests in addition to a comprehensive exam:

CBC
CMP
TSH
Free T4
HbA1c
Ultrasound Thyroid/neck

Based on the results of these, an Ultrasound Guided Fine Needle Aspiration (UGFNA) procedure may need to be considered for biopsy/cytology.
