Question : What should I do? About 3 months ago, I was urinating and experienced a tightness in my right testicle/ perineum area. I checked with my family Dr., and was notified I had chlamydia. Anyhow, I got treated, and was eventually cleared. Right after the tightness scenario I underwent, my right testicle constantly had felt in the way, never descended lower than the left testicle as it used to always do, and always feel a dull ache/tightness. Especially when I am sitting down, I feel tightness in the right testicle. It worries me that it is always up while the left hangs. I know that people say it's normal for one to be up higher than the other, but 3 months ago, I never felt this way. And it wasn't always this up. I did go in for an ultrasound, which resulted in nothing being found, besides a benign cyst sitting on the top of the right testicle. I still don't think this is what's causing all of my problems because I'm sure that benign cyst had been there before the 3 months and I never felt this way. Even till this day, when I sit, or even stand, I feel tightness, a dull ache, and I am very annoyed and concerned about it all. This issue is also very much affecting my ability to comfortably engage in sexual intercourse, because my testicle is in the way and being pushed against. I also had to go to the ER one night because the pain was worse since I had been sitting driving for a long period of time. I also began feeling nausea in my abdomen. The ER Dr. said it could be an inguinal hernia. I met with a specialist general surgeon who manually felt and said he didn't think it was an inguinal hernia after examination, but said if I feel it still hurts in 6 weeks, to continue taking my naproxen and to visit him again. But this problem has been there for months so I don't suspect for another 6 weeks to help me out. It feels heavy, dull, and in the way. And when I have sexual intercourse, since it's snug so high, it's hurting and aching me. I also feel it worse while sitting down, laying down, or putting pressure sometimes on my lower back. What is the next step I can take? I am not sure what it is myself, so I cannot really explain it to the Dr.'s myself. The only medication I was prescribed was Naproxen 550mg to be used as needed. This hasn't helped me in the least. Please advise me as to what to do. I wonder if there is any scans or anything that can be done that can pick up something that an ultrasound can't, to perhaps find a physiological issue? Please help. Thanks.

Thanks for your query,based on the facts that you have posted it appears that you have pain and discomfort in your Right Testicle and have noticed it to be placed at higher level as compared to Left one.

At the outset I would like to state that the position of testis as long as it is in scrotal sac has no significance at all.It is natural for every individual to have one testicle lower than its counterpart.The pain and discomfort that you have could be due to chronic infection of the spermatic cord (Funicullitis).mostly due to Chlamydia infection.This pain causes reflex contraction of cremastic muscles resulting in to pulling up of your right testicle.

Please repeat your ultrasound scanning and colour Doppler study of scrotum to conform the diagnosis with specific reference to the spermatic cord.

In the mean while start taking antibiotic like Erythromycin along with anti inflammatory drug like Diclofenac twice daily.You may need to take medications for 3-4 weeks.

Your symptoms are aggravated by your anxiety .Please try to calm down and note that it is the function of the testicle that is important than position of testicle.

You can try to use loose underwear to avoid pressure on testicle that will help to relieve compression on the testis and there by reduce pain and discomfort.

Thank you Dr. for the follow up. Sometimes, I also have a dull feeling or also numbness feeling associated with this testicle. It scares my to believing it may be dead or something, due to low blood flow. Is this something that can happen with the condition you explained to me, Funicullitis and cremasteric reflex. I'm worried that my testicle having this problem for so many months can cause some long term damage, as at times, my testicle feels dead or numbish. Also, how can I go about getting an Erythromycin prescription and would Naproxen 550mg work? I currently am taking Naproxen 550mg as needed, but do not have access to Erythromycin and don't even know the strength that I would need? Is there anyway to get a prescription for this online as I'm not sure how I would get my family Dr. to prescribe me an antibiotic. He only gave me two tablets of azithromycin and cleared me of the chlamydia when I got tested, and I also believe that the inflammation and bacterial infection of the spermatic chord may still be a problem despite the "Clearance" of the STD. Please help me out as to what I should do next.

Testes never gets damaged with such a mild infection .
It is your anxiety that is bothering you.

As regards prescription for Erythromycin we can not prescribe any drug unless we have carried out physical examination of a patient.

I think Azithromycin is available in U.S over the counter ,Since Azithromycin is derived from Erythromycin you can continue taking it twice daily for 10 days.

You can take Brufen twice daily .It is available over the counter.

I guess I must request to get antibiotics from my PCP as no antibiotics are OTC in the US that are in tablet form. So, can this inflammation of the spermatic chord still be present even though I am cleared of chlamydia? Or does this mean even though I was cleared, I still may be infected with chlamydia? Sometimes, the right testicle even feels as if it has a lack of sensation which really worries me. And during intercourse, it pushes in the way and really begins to ache me even worse, and the other has no signs of this. I am wondering how come none of this was told to me by my Dr. when I was given azithromycin for one day to cure the bacterial chlamydia infection. Also Dr. , just to be safe, I will schedule an MRI, since there is right side abdominal and right side waist ache/pain associated with this right testicle pressure. Is there anyway this MRI can show infection, swelling, or the root of my problem? In other words, will getting an MRI of the area be helpful in figuring out something that can treat me? Thanks for follow up.Doing MRI will not serve any additional purpose .You need to take medications religiously for 10 days .

Pain in groin and lower abdomen are due to Funiculitis .

All other symptoms are due to apprehension and anxiety.

Contact your PCP and get the prescription from him.
