Brief Answer:
autonomic nervous system activity

Detailed Answer:
Hi,
I went through the query and understood your concern.

In some people during or after micturition there may be shivering or tingling sensation. This is due to autonomic nervous system activity. No need to worry for this as this is normal phenomenon.

But since you had cardiac surgeries, you should monitor your BP, lipid profile, sugar and also continue antiplatelet (blood thinner) to prevent any problem.

I hope this information answered your question. Feel free to ask for any further queries.
Wish you good health.

Regards
