Question: Hi Doctor,

I was in Brazil last month and had sex with 4 prostitutes on May 12,13 and 25.

I used protection all of those time, and after removing, checked for any tears too, and found none, no slippage as well.

I had this weird tingling sensation in my throat and acid re fluxes starting from XXXXXXX 1.

I immediately saw a doctor who said its just an infection and gave me Hexamedine oral spray. Did not make a lot of difference.

I got tested on XXXXXXX 3 (7 days after the last sex encounter, May 25 and 18 days the first sex encounter, May 12&13).

All that exists in STD, from gonoreha to HIV 1/2, and used the 4th gen duo test methods. Everything was negative.

I was not satisfied with my throat condition, so I saw another doctor (General Physician), he noticed white discoloration of my tongue and said it was tonsillitis, and said I was not drinking enough water, which was true at that moment.

I am a 27 yr old man, now I am not sure whether there are tonsil issues at my age. He did another test on XXXXXXX 10, for H.Pylori in blood and that too was negative.

Went to the doctor again as i noticed some swelling infront of the right ear, he confirmed it was a swollen lymph node. I told him about the sex encounters and told him I did went down and licked the Vagina. After listening to that he was little concerned, he said although there is very little chance of oral transmission but so far its negligible, but he was not sounding very confident.

He gave a shot of steroid to bring the swelling down, and told me not to worry as to him it appears an allergy.

My question I still have that white tongue, with little swelling on the right front ear node, no fever, no pain, nothing else, should i be worried.

Thanks for posting your query.

You had all your intercourse as well protected and you were sure that the condoms were intact till the end. That it self assured you that your risk for HIV is negligible. Oral sex carries a very low risk for HIV/STD. That too oral sex with a female has lesser chance for STD/HIV compared to oral sex with a male.
Earliest HIV manifestation can occur 4-6 weeks after the risky contact, if there was any infection. Not earlier than this. So I am sure that your symptoms were not related to HIV.
So your throat infection and the lymph node swelling are most likely to be ordinary throat infection which can occur to anybody. Tonsillitis can occur in you age though not common. If the antibiotics were not very much appropriate the symptoms might persist.
A throat swab for both non specific and Gonococcal culture and sensitivity and treating the same as per the outcome of the test result will clear your problem soon.
Your tests (4th generation) done 7 days after your last encounter is no way conclusive.
But as per your statement the risk is negligible only.

Thank you doctor, as you said that the test were in-conclusive at 7 days, but there were 2 sex encounter:
May 12 and 13
May 25
Tested on XXXXXXX 3: Does this test at least clears any shadow of doubt from the sex encounter from May 12 and 13? Thats like 20 days!
XXXXXXX 24: Enlarged lymph node in-front of ear(which was painful), also some sort of stiffness(maybe swelling) exactly under the ear the between jawline both sides(non-painful), looks to me like a swelling. Are there any lymph node glands exactly under the ear?
On Jun 24, the doctor took another full sample to test for everything on my request. I am still awaiting results, but my fear are the worst, and I am not able to sleep well in nights. Although your assurance is very helpful. But, its just that the timing doesn't fit.

My problem, I have a white hairy tongue followed by tonsillitis, which I never had in life, also the painful swollen node in front of ear, stiffness under the ear non painful.

When do swollen nodes appear after exposure, do they swell all through out the body or just one location?
Do you still feel there is no need for me to worry, how conclusive are the results going to be from Jun 24?
Can you please let me know what other things/symptoms should I keep an eye out for, because if it doesn't happen, I can calm myself and have some confident that my fears are not true?

Lymph node enlargement under the ear lobe indicates the involvement of Tonsil. Stiffness in opening the jaw is quite possible with pre-auricular lymphadenitis. These are all just sequence of an ordinary throat infection.
They are not related to HIV. In HIV painful nodes can be present as a localized one and painless bilateral enlargement at multiple sites can also occur.

A negative 4th generation test done after 4 weeks can eliminate HIV 1 infection conclusively. If it covers HIV2 ANTIGEN then it is more value. Usually HIV antibody test has both 1&2 in 4th generation test.

I did request both of them.

To clarify opening the jaw is not stiffening at all. There is a non-painful swelling below both the ears and between the jawline, and one painful node infront of right ear, as you said.

Based on my symptoms and timeline so far, in your expert opinion should I be worried about the results?

The painful lymph node enlargement in front of you right ear has many reasons from a furuncle over external ear to a scalp infection. Unlikely to be related to HIV.
Your physician will be examining the all drainage areas for that lymph node and with appropriate oral antibiotics he will be able to offer you a full relief.

Thank you doctor, that's the trouble so far my doctor has not inspected all that.

Also today morning I am feeling a very discomforted in the lower end of my throat, like something is choking the airway, also my naustrals are slightly blocked, so far no phlegm, but i did notice some blood in the morning spit.
