Question: what is the diagnosis for a patient presented with failure to concentrate, uncontrolled muscle spasms in arms and legs, frequent falls, trouble swallowing and pronouncing common words.

It is difficult to achieve a correct diagnosis without visiting the patient and with only these symptoms, however, it is clear that neurological disorders can cause such symptoms.

Conditions to consider are seizures, stroke, Chiari malformation, Myasthenia gravis, etc.

Neurological examination and tests (imaging studies, EEG and other blood tests) are necessary to achieve a correct diagnosis.

If these neurological conditions are excluded, psychiatric disorders such psychosis should be considered.
