Brief Answer:
Stone in bile ducts.

Detailed Answer:
Hello!

Thank you for the query.

Hiatal hernia usually does not cause upper right abdominal pain. So your pain is most likely caused by bile ducts problem. After gallbladder removal there is a possibility that some stones are left in the bile ducts. In such case, the symptoms can be similar to those present with gallstones. It gives upper right abdominal pain and back pain along with nausea.
Hernia usually gives upper middle abdominal discomfort and burning. The pain does not radiate to the back.

I suggest you to consult your doctor. Abdominal ultrasound, blood work, liver tests, pancreas tests should be done. If nothing will be found, gastroscopy should be next.

Hope this will help. Feel free to ask further questions.
Regards.
