Question : hello doctor, me and my girlfriend nearly had sex, i used condom but did not injected in her vagina, but we feared a lot, so she took postpone 72, after 4 days her vagina starts to bleed, what do we do now ?

Thanks for writing to HCM.

The emergency pill (post pone 72 pill) works better when it is taken within 72 hours and has a 85-90% success rate since your girl friend has also taken it immediately.

This pill interferes with the hormonal balance of the body as it contains high doses of hormones and the bleeding could be a result of this hormonal imbalance resulting in a withdrawal bleed.

You may wait for few days to subside this bleeding and see if she continues to bleed,you should consult a gynecologist for an opinion.

Otherwise, you can wait for the next period to set in as per the dates and if that does not happen, consult your doctor within 2 weeks after the missed period for excluding pregnancy (chances are very less).

Emergency contraception should not be used frequently as it is going to disturb the normal physiological function of the body. Also Periods can get disturbed and become irregular for sometime when emergency contraception is practiced.

But we didn't had sex at all, her period just got over my 22nd XXXXXXX and she took the pill on 26th XXXXXXX she feels like pain occurs while on her period right now , can she take any medicine to avoid the period pain ??

Thanks for the reply and sorry for the late response since I was quite busy in the morning.

If you had not sex then there is no chance of pregnancy and this bleeding is due to the effect of that pill as we discussed earlier.

She may take over the counter pain killer like ibuprofen 400 mg twice daily for symptomatic relief otherwise there is no need to worry.

Her periods may get disturbed for couple of months because of pill but it will become regular after that and do not take pill when you have not had sex or whenever there is protected sex.

Thanks for your valuable answer doctor, but I can't able to get my penis in to her vagina, it's too tight and she feels a pain a lot, so what is the solution for that ? That's why we didn't had sex.

Welcome again,

I have gone through your query again and understand the concerns.

This kind of problem may arise during intercourse mostly because of partner, when she is probably in extreme fear of pain so when you try to get in, the opening of vagina and surrounding area goes in to spasm.

This leads to narrow vaginal opening making difficult for penis to penetrate deep inside it.

At first, I would advise the use of a good and sterile lubricant which would assist the penis in entering the vagina with more ease.

Also, there are multiple devices (called as Vaginal Dilator Set- Natural Shape) available in special pharmacy stores (and also available online) which may help in the dilation of the vaginal entrance.

They come in different sizes and need to be used for a certain amount of time on a daily basis to help achieve dilation.

I would also recommend to use your fingers with the application of a lubricant like KY Jelly or Vaseline and insert them into the vagina. Do not try to insert them dry.

You can follow this steps to have a complete penetration.

1st - try to relax your wife and be with her in naked position without attempting penetration. You should wait till she starts enjoying your touching and rubbing but don't touch genital areas immediately (which might cause reflex contraction of vaginal muscles).

2nd - then add chest area and pubic area to the range of touching and massaging.

3rd - try insert single finger with the help of KY Jelly or Vaseline, and move it in and out.

4th - slowly make it two and when it is easy, try and insert penis.

By this protocol, you may achieve full penetration and if still problem persists then you may need sexologist personal visit.
