Brief Answer:
Local lesions should be ruled out

Detailed Answer:
Hi dear, I have gone through your question and understand your concerns.
If the vaginal insertion of any pessary is causing vaginal bleeding, then a local lesion in cervix or vagina can be a cause.
Local lesions like polyp, cervical fibroid, or precancerous or cancerous lesions.
I will suggest you to consult an OBGYN specialist to get properly examined internally, investigated including pap smear and get properly treated.
Hope you found the answer helpful. Please do get back for further queries.
Wishing you good health.
