If your urine pregnancy test has turned out to be positive and the ultrasound doesn't show any sac in the uterus and moreover you have started bleeding as well per vaginum which is likely to be a darkish brown spotting , then there is a possibility that this may be an ectopic pregnancy, possibly inside the tube ( fallopian tube).
A repeat ultrasound transvaginal is advised with an aim to rule out ectopic pregnancy and a serum beta hcg level test should be done to reconfirm the hcg levels which if more than 5ng/dl then you are surely pregnant and if pregnancy is not in uterus then this could be an ectopic for sure which can be an emergency.
So please get repeat targeted ultrasound and beta hcg test done asap.

Hello,
The hcg levels double in every 48 hrs in an ongoing developing viable pregnancy ,Generally the gestational sac is visible when the hcg levels reach 1500 +, hence the doctor wants them to eb done every 48 hrs so that he/she can schedule a repeat transvaginal ultrasound to locate the sac. Because if located inside uterus then you are safe completely and its not an ectopic else you may need a laparoscopic salpingotomy or salpingectomy to remove the tubal pregnancy.
If the hcg levels are decreasing and reduced more than half now, it indicates a failing pregnancy or a chemical pregnancy but still it is necessary to localise the pregnancy as any pregnancy in tubes can be an emergency as said earlier for fear of rupture.
You may get a repeat hcg after another 48 hrs and then a repeat targeted USG for localising the decidual reaction or gestational sac.
