I have read through your query in details.
Please find my observations below.
1. It appears that the spleen is probably enlarged and causing pressure effects and this is the reason for pain. This pain is visceral pain due to pain involving and less likely to be controlled with a single non steroidal OTC pain reliever.
2. The pain from an enlarged spleen usually requires more than one agent and therefore the agent should be able to act on particular neuro receptors and reduce the pain. Some of the medicines are issued on prescription only after consulting a doctor.
3. I suggest you include OTC pain relievers with anti spasmodic combination and use them as recommended.
