Question: I have had recurring varicose veins and a total of six varicose-vein strippings in my right leg between 1954 and 1989. Currently, vein clinics on the West Coast and the East Coast claim I need more varicose-vein treatments, which I adamantly reject. Regardless, here is my question: Why have I never had visible signs of ulcerations in my right leg? Could the answer be that my superficial venous system has been obliterated?

Hello!

Thank you for the query.

Ulceration is the last, most severe step of veins insufficiency. There must be strongly developed veins disease with superficial thrombosis, high pressure in veins and small veins circulation disturbance.
As you have had superficial veins removed many times, you have never reached to this most advanced point. And it is possible you never will.
Please note that not every one who has varicose veins will have ulceration.

If its about another varicose veins surgery, please note that you may protect your leg without this surgery if you have decided to not have it. Compression stockings used regularly can prevent veins disease progression.

Dear Dr. Stanko: Thank you for your answer which indicates I am at an "early stage" before ulceration could occur. I take issue with your diagnosis and support my contention that I have not had ulceration because my right leg superficial venous system has been obliterated. Accordingly, to support my medical contention, I will quote from the National Institute of Health (NIH) Pub Med abstract
J vasc surg 1993 Mar; 17 (3) : 487-90:

"In just over half the patients with venous ulceration, the disease is confined to the superficial venous system. This group of patients may benefit from surgical treatment..."

Surgery is effective in ulcer prevention.

I would say that superficial veins system malfunction is responsible for most of the venous ulceration. And that is true that surgical treatment is very important in preventing ulceration. This mostly confirms what I have written above.
I just wanted you to know that there is an alternative which can be also very effective. This alternative is compression.
