Question : My mother age 72 years, hypertensive for last 12 years or so, is currently on Amlodipine 5mg and telmisartan 40 mg. She is , for the past many months complaining of weakness, not being able to do her household work, fatigue and lethargy. We have got her Blood tests done life KFT,LFT,TSH, Haemogram, Lipid Profile, Blood Sugar etc. We found her Uric acid at 6.90 and and LDL at 148. Her Haemoglobin is also at 11. She has been prescribed medicines like Febustat 40 mg for uric acid, Rosuvastatin 10mg, Mecobalamin + Folic Acid tablets. Her doctors have examined her thoroghly but unable to do anything for the weakness feeling which is now becoming frustrating for her.
Last year the doctors had put her on Eslo/Eslo D for her BP. But over time she started to feel very weak and we changed to Amlodipine &Telmisartan recently, but with not so great results in controling her hypertension. It remains in the range of 145-160/75-80 Pl advise.

Looking at the clinical history and query, my opinion is as follows:
1. The cause of weakness could be
a. Anemia
b. Chronic inflammation - raised ESR indicates chronic inflammation. - Does she have any fever, joints pain, etc.
c. Her total leukocyte count is less and her neutrophil is also mildly reduced, - indicating she might be having chronic inflammation.
2. The blood pressure is varying a bit. However, at this age, a BP of 140/90 is well controlled.
3. To improve weakness she can follow:
a. Her diet needs to watched for. She needs to have regular smaller quantities.
b. Regular sleep.
c. Mild walking/exercises
d. Family involvement with increased social activities.
Do get a CRP (c-reactive protein) test. Her ESR is high indicating inflammation.
Are any lymphnodes enlarged?
