What Causes White Spot Around Base Of Penis Glans Post Unprotected Sex?

Posted on Thu, 30 Oct 2014
Question: I am a 36 yo XXXXXXX (South Asian) male living in the USA. I've been having unprotected sex. I have never had pearly penile papules. 6 months ago I developed a new white spot around the base (sulcus) of the glans. It was tiny at first, just a raised white bump, and it slowly grew, and the shrunk again. I now have two new ones. One is large and visible to the naked eye. The other two are very hard to see. Once I become erect they are much hard to see. They are completely painless. I have gone to two doctors and gotten lots of STD panels and come out negative in all tests (herpes, ghonerea, chlamydia, hiv). I just want to know what these are. Any ideas?

Hello and welcome to healthcaremagic

I am Dr. Kakkar. I have gone through your query.

I would request you to upload a better photograph (may be 2 or 3 photographs) which clearly depicts the spots you are concerned about. Click one in natural light with flash off. In this photograph, I am not able to see the spots that you mention.

Regards

I'm attaching two more pictures. I apologize if they are still insufficient, it's really hard to get a good picture of it. I can keep taking more until you have a clear picture of what you need. All the light I have is incandescent. In white neon light it is much more visible.

To help you understand it more, it is shaped like a thin white line, running the same direction of my penis. It's covering almost the entire length of the base (sulcus) of the glans. It's been there for 6 months now. It's not a pimple full of pus, but looks like one.

Brief Answer:
Possibility of Lichen Nitidus

Detailed Answer:
Hi.

I have reviewed the latest photographs. What I can see is a hypopigmented spot, which is most likely a post-inflammatory hypopigmentation/possibly secondary to Lichen Nitidus. However, I would like you to upload a few more photographs

This is not an STD by the look of it.

Treatment options are topical steroids and topical immunomodulator e.g tacrolimus.

If I was the treating doctor I would have asked you to use a topical immunomodulator e.g Tacrolimus 0.03% ointment, twice daily for a few weeks.

It is a prescription drug and I would suggest that seek and appointment with a doctor, preferably a dermatologist, who would be able to confirm and also write a prescription for this
