What Causes White Tongue After Drinking Cranberry Juice?

Posted on Wed, 2 Apr 2014
Question: Good Afternoon Doctor : Since early December I have been having a " white tongue ". I am not sure if there is a relation but it began by a mild burn/ irritation on the top back of my tongue some hours after having drinking a large amount of cranberries juice. A couple of days later I was experiencing burning in my throat and esophagus. During a time my mouth felt like I had eaten raw pepper , I was blowing hot air . I did many blood and urine tests , a gastroscopy , an MRI of the upper torso ( see attached reports ) and so far not a single doctor " specialist " has been able to diagnose the cause of my symptoms or offer me a way to reduce them. Over three months , the only noticeable change is that I am not blowing any more " spicy air ". The tongue remains white and I have noticed that whenever my saliva PH is low (5.75 ) it is when my tongue is the most "chalky white ". As of today I am still struggling to find out if my condition is related to a gastric problem or a viral one or an ENT disease . Looking forward to hear your opinion and if you need more precise information such as daily diet , time of meals , medicine , symptoms at different times of the day I have all that written down. Thank you XXXX

Brief Answer: Laryngopharyngeal reflux Detailed Answer: Hi Sir, I have gone through your query and in my opinion you may be suffering from the laryngopharyngeal reflux. For your information drinking cranberry juice increases the amount of salicylic acid in the body and salicylic acid acts as keratolytic agent and damage the mucosal layer of the skin. This may trigger the symptoms in association with laryngopharyngeal refux disease. Therefore I personally advised to avoid cranberry juice, steroid drugs such as methylprednisolone / dexamethasone, avoid smoking and alcohol drinking if you have and also avoid spicy food. You may want to consult ENT specialist for better diagnosis and management. If you have any doubt, I will be available for your query.

Thanks Doctor but I only drank cranberries juice once ( Dec 05 /06 ) . Symptoms came after and I was just guessing about the juice having caused me my condition that has been going on for 3 months now. I never drank alcohol nor smoked in mi life. Do not eat any spicy foods , no fried food , no red meat . Not over weight and do exercise regularly. Never seen a doctor in my life until last December . Would you be able to explain that as soon as I swallow saliva or drink or eat I immediately feel small bubbles up my throat ? I mean there is no delay like waiting for the stomach to begin churning... This does not happen systematically all the times I swallow but I noticed it happening on and off during the day and at night laying down. It is like having a " nervous squeeze " in my throat... ( my throat was checked twice by an ENT via small wired camera through the nose and 2 throat cultures were performed and came negative ) Thank you, XXXXXXX

Brief Answer: Consult gastroenterologist Detailed Answer: Hi Sir, I have gone through your follow up query; and these symptoms related with above mention disease laryngopharyngeal reflux but if there are no sign of this disease after consulting with the ENT specialist two times then better to consult the gastroenterologist to rule out any disease related to his/her specialty. Thanks
