Question : kindly suggest is there any complication of CKD and Sever Metabolic Acidosis due to chronic alcoholism.

Hepato-pulmoray syndrome and hepato-renal syndrome due to cirrhosis

Detailed Answer:
Hello,
1) Chronic alcoholism + breathing difficulty as a chief symptom suggest patient had either Hepato-pulmonary syndrome or pulmonary hypertension secondary to liver cirrhosis.
2) It is unlikely to have CKD in chronic alcoholism. But patient develop Hepato-renal syndrome in liver cirrhosis.
3) Chronic alcoholism is causative factor for liver cirrhosis in this patient.
4) This patient would have hepato-renal syndrome related kidney shut down which ultimately lead to anuria (urine output <50ml in 25hr) and ultimately metabolic acidosis.
5) Severe hepato-pulmonary syndrome can also lead to acidosis due to poor exchange of gas between lung and blood but this lead to respiratory acidosis not the metabolic acidosis.
