Hi,
There are few possibilities: either pyoderma which is bacterial infection of the skin or eczematous rash.
I need to ask you few questions:
What is the duration of the lesions?
Is the rash itchy or associated with pain and burning?
Is it present on one side of the face or both the sides?
Has he used or applied anything on the face and ears?

it's on both sides and itchy, burning sensation
started out red spot three days ago and got worse

Infected eczema, steroid antibiotic cream, oral antibiotics, antihistaminic

Hi,
According to your complaints it seems to be infected eczema.
If I was your treating doctor I would recommend you to apply steroid antibiotic combination cream like Mometasone cream and Fusidic acid cream on the affected areas twice daily and take oral antibiotic like cephalexin 500 mg twice daily for 7 days.
For itching take antihistaminic like tablet Cetirizine 10 mg once or twice daily depending on the intensity of itching.
Antibiotics and steroidal preparations are prescription drugs and you need to see the doctor in person to get them.
