Brief Answer:
Advice to undergo detailed neurological evaluation

Detailed Answer:
Hello,
Thank you for trusting us with your health question.

I have gone through your medical history in detail. Fatigue and lack of motivation can be associated with hypothyroidism and depression. However, since you are already on treatment for those conditions, I believe those are adequately managed and you are undergoing regular follow ups.
If the testosterone levels are within normal range for your age then it is not of much concern as discussed by your primary provider

However, since you are complaining of loss of balance along with other symptoms, neurological disorders like Parkinson's disease need to be ruled out.
A detailed neurological evaluation is needed at this point. Depending on the initial screening brain imaging like CT/MRI scanning may be needed. So please be advised to follow up with your primary provider who can refer you to a neurologist for detailed evaluation.

Hope this answers your question
