Question : Hi, I have developed hemorrhoids (piles) after the birth of my daughter in 2011. It's been there all these years and I was taking some homeopathic medicine my father had given me to keep it under control. Unfortunately, he passed away last year and I am not doing anything for it. For the last few days, it has been troubling me a lot, It's getting painful with itching, burning etc. I wanted to ask if there is over the counter medicine I can get in the US (preferably online) so that I can get relief? Also, what is the long term treatment for the problem ?

First, from this photo, it can be seen that you have internal hemorrhoid and anal fibroma. These are 2 different issues. Hemorrhoid may bleed, cause pain during defecation and fibroma may cause itching and pressure feeling. Local ointments and cream can relieve symptoms but won't cure the cause. I can recommend Butlers hemorrhoid ointment and Naticura Hem-Control pills which may lead to improvement. However, for permanent solution you should consider rubber Ligation or laser therapy.
