Question: I am a 54 year old women. Been having headaches and nosebleeds for about a week. The last 2 days having pain in right chest that radiates under my arm pit. What could it be?

Regarding your concern, I would explain that the headaches and nosebleeds could be related to uncontrolled high blood pressure.

Have you measured your blood pressure values during these episodes?

The chest pain that radiates under your armpit should be differentiated between a musculo-skeletal pain and coronary artery disease.

For this reason, I recommend going to the ER for a physical exam and some tests:

- a resting ECG and cardiac enzymes
- complete blood count, PCR, ESR for inflammation
- kidney and liver function tests
- blood electrolytes
- a chest X ray study.

An exercise cardiac stress test may be needed if suspicions of coronary artery disease are raised.

In the meantime, I recommend you to closely monitor your blood pressure values in the next days and refer them to your doctor.

Thank you for your response. BP normal 118/70...I have had a cardiac stress test done about 2 years ago. Everything was normal..

Although your exercise cardiac stress test was normal two years ago, coronary artery disease can not be excluded now.

For this reason, I would recommend performing the above mentioned tests.

Regarding the bleeding nose, I would recommend performing coagulation tests, inflammation tests (PCR and ESR) and thyroid hormone levels for thyroid dysfunction.

It is also necessary to closely monitor your blood pressure values during those episodes( if they occur again) for possible fluctuations.

You should discuss with your doctor on the above issues.
